Job Description

Purpose of Position: Manage potential risks and liabilities within the Department of Psychiatry (inpatient and ED). Monitor and ensure implementation of corrective action plans. Provide written reports as requested.

General:

Collaborates with Site Director of Behavioral Health in clinical and risk related duties Responsible for the receipt and review of all Behavioral Health (BH) VOICE occurrence reports and allegations. And enters all BH incidents into NIMRS system. Initiates investigation of BH sentinel events and other adverse events including, chart review, video review, interviews of staff and patients. Responsible for OMH/JNY Justice Center (NYJC) investigations which include:

Allegations of Abuse/Neglect, significant incidents and non-NYJC incidents Respond to the NYJC 72 hour requests for information, video review, chart review, staff and patient interviews, completion of reports to NYSOMH/NYJC with findings within specified timeframes Development of Corrective action plans, Measures of effectiveness Conducts through investigations and review of all reportable and non reportable BH incidents Participates sand prepares investigative reports for IRC and SRC committees and RCAs Submits investigation reports for allegation of abuse/neglect in NYJC web Submission of Investigation Report system (WISR) Provides required follow-up for Justice Center onsite and mail investigations, including submitting requested medical records, incident reports, staffing sheets and videos Submits all administrative actions for substantiated findings of reportable abuse/neglect to the Justice Center Administrative Action Reporting Mechanism (AARM) system Gather risk related data and enter same into computer software systems as directed NIMRS, NYPORTS, Etc.,) Reviews and enters all psychiatry related incidents into software system NIMRS, identifies trends and patterns for further intervention and suggests corrective actions as necessary Monitor and review corrective action plans ensuring compliance with required actions/interventions Assists in the preparation of risk assessments for the behavioral health division Attends all 'Behavioral response Team events (BHRTs; review video of same and investigates as required Assists in review and monitoring of all BH seclusion and restraint instances including video when available Participates in ACT/OPD OMH regulatory visits Participates in CMS Title 18 Allegation survey, or other regulatory agency surveys (TJC) as assigned Assists in completion and investigation of NYPORTS incidents and enters information into HCS system as required Monitors and ensures evidence and implementation of corrective action plans and risk reduction strategies for the behavioral health inpatient and ED units Assists in performing environmental safety rounds (readiness rounds)

Provides reports to leaders as required

Minimum Qualifications 1. A Master's Degree in Art or Science with a specialization in Hospital Administration, Health Care Administration, Administrative Medicine or in Public Health when conferred for a program in hospital administration from an approved college or university; and, 2. Four years of administrative experience and background at responsible management and administrative levels; or, 3. An equivalent combination of training, educational background and experience in related fields and educational disciplines; and, 4. Knowledge of fundamentals of hospital organization, administration and standards, regulations and laws applicable to hospital operations. Knowledge of principles of business and personnel administration, management functions and support service functions and ability to direct and supervise personnel.

Department Preferences

Experience in Risk Management or clinical investigations, able to analyze and manage risk management issues

Licensed healthcare professional preferred (RN, NP, PA, PhD)

Excellent writing skills required, proficient in MS office

Demonstrates attention to detail and accuracy

Able to work independently

Training and experience in conducting NYS Office of Mental Hygiene/Justice Center investigations
