SUMMARY As a Community Health Nurse, your role will focus on delivering nursing interventions, patient education, and care management to diverse clients within our community. This essential work promotes good health and allows you to provide personalized care in the comfort of their homes. Per Diem LOCATION:
Wayne, Seneca, and Ontario counties. DEPARTMENT:
Home Care SCHEDULE:
Days/Evenings, Saturdays & Sundays. Orientation would be M-F for 2-4 weeks. ATTRIBUTES Compassionate, independent, and patient-focused. Experienced in comprehensive nursing assessments, documentation, and employing decisive judgment in professional behavior. Two (2) years of home care experience preferred. Bilingual ability preferred. Minimum of one year nursing experience, including IV and wound care experience. RESPONSIBILITIES Patient Care & Service:
Conduct comprehensive assessments of acutely ill clients in their homes; provide skilled nursing care based on physician orders; educate clients and families according to the care plan. Planning:
Develop and document tailored care plans to meet each patient's unique needs, collaborating with the interdisciplinary health team as needed; maintain clear communication regarding patient health status and treatment plans. Communication:
Ensure consistent communication about patient health status, treatment plans, and progress; collaborate regularly with office and supervisory staff to coordinate timely client services and referrals. Documentation:
Accurately document all aspects of client care and implement physician orders following program standards. Compliance:
Adhere to all required department protocols, regulations at local, state, and federal levels, and fulfill educational requirements. EDUCATION: AS: Nursing (Required) LICENSES/CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Requires exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; frequent walking, standing, or squatting required. PAY RANGE: $73,528.00 - $98,072.00 CITY: Newark POSTAL CODE: 14513 The stated pay range is a good faith representation of potential base pay for a qualified full-time candidate. This may be modified in the future and is eligible for additional pay components. Compensation depends on various factors like experience, relevant qualifications, specialty, internal equity, location, and contracts.
