Job Summary:
The HBPC Primary Care Physician provides comprehensive, primary medical care services, with an emphasis on optimizing functional status and quality of life, for patients who are not best served in a traditional office setting. The ideal candidate will have a passion for providing comprehensive health care to homebound populations in the context of their social support systems, physical environments, and personal values. The HBPC Primary Care Physician role is a 0.6 FTE time commitment and can be combined with other positions/responsibilities for up to 1.0 FTE role in partnership with the Department of Family Medicine & Community Health, Division of Geriatrics and Gerontology, or Division of General Internal Medicine.
This position will have clinical responsibilities with the HBPC interdisciplinary team based out of an office at UW Health Administrative Services and will require travel to provide care to patients in private residences, senior apartments, assisted living facilities, and skilled nursing facilities. As a clinical faculty member, this position will also include teaching responsibilities, active engagement in interdisciplinary teamwork and quality improvement activities, a commitment to self-directed learning in the home-based medical field, and participation in other education and training activities as recommended by the HBPC Medical Director. Responsibilities: - Provide comprehensive, interdisciplinary, primary care in the homes of older adults with serious medical, social, and/or behavioral conditions for whom routine clinic-based care is not effective or may not be possible. - Maintain and continually develop foundational knowledge related to chronic disease management, geriatric syndromes, dementia care management, palliative care, caregiver support, and home- and community-based services. - Work in conjunction with HBPC interdisciplinary team to determine in- home comprehensive geriatric needs and to ensure responsiveness and continuity of care. - Work together with Advanced Practice Providers in a collaborative practice model for ongoing management of shared patients. - Provide on call patient care on a rotating basis. - Educate patients and families on goal-based, appropriate preventative care, medical issues, and use of prescribed medications and treatment. - Provide compassionate anticipatory guidance through chronic illness trajectory, communicate prognostic information, and engage patients/families in goals of care discussions with referral to hospice when appropriate. - Provide clinical teaching to medical students, residents, fellows and and interprofessional learners about home-based medical care. - Collaborate with faculty and staff from the Office of Population Health, Geriatrics, Palliative Care, Internal Medicine, Family Medicine, and Ambulatory Operations to enhance and continually improve care of patients receiving HBPC services. Coordinate care with appropriate clinical team members in the inpatient, emergency department, and subacute rehabilitation settings. It is anticipated this position requires work be performed in-person, onsite, at a designated campus work location.
