Eagle Crest Communities is seeking a Nurse to join theHolmen Campus serving the residents of our 12-unit memory care home and 16assisted living apartment community.
The successful candidate(s) will partner with the campusmanager, provide direction and leadership to our team of dedicated and hardworkingResident Care staff.
What You'll Do Communicate with residents, families, andproviders, in the coordination of care Actively maintain Individual Service Plans andResident Risk Agreements Responsible for oversite and provision ofmedication management and treatment services, physical and behavior assessmentsor the residents Duties will be performed in compliance with CBRFregulations (DHS 83) and RCAC regulations (DHS 89) and policies and bestpractices established by Bethany Lutheran Homes, Inc What You'll Need
Must be licensed to work in the State ofWisconsin as a Licensed Practical Nurse or Registered Nurse 1+ year in a position that provides oversiteregarding medication administration and clinical duties for direct care staff Experience working with geriatric populationand/or memory care is beneficial, but not required-we will mentor those who fitwith our culture and core values! Drivers require a valid driver license and musthave and maintain a safe driving record Our Wages
