Position Overview: As a Licensed Practical Nurse (LPN), you will play a vital role in enhancing the lives of our residents by providing high-quality nursing care within our supportive team environment. Your primary responsibility will be to assist in all Nursing Department functions while adhering to established policies and procedures. Shift Options: We offer flexible shift arrangements: full-time, part-time, or PRN with 12-hour or 8-hour days, as well as 12-hour night shifts, including rotating weekends. Key Responsibilities: Conduct daily nursing functions in compliance with current regulations and guidelines. Supervise nursing personnel to ensure adherence to policies and procedures, including safety standards. Manage resident admission, transfer, and discharge processes, documenting care and services provided. Participate in the orientation of new residents and their families. Conduct rounds with healthcare providers. Utilize the skills of nursing staff according to residents' care plans and healthcare provider orders. Complete daily rounds to evaluate resident care and report any significant findings. Address issues raised by residents and family members, escalating as necessary. Assist in facility surveys and inspections when requested. May oversee specific clinical programs, such as dementia care or infection control. Qualifications: Active LPN/LVN license. Experience in home care, assisted living, or long-term care. Strong independent decision-making skills, ability to follow instructions, and openness to feedback.
