Community Physical was founded in 1986 by Bob Tripicchio, PT, MS, DSc, on the premise that people are the most important asset of any organization. CPT was devoted exclusively to the delivery of home care services until 1996, when we expanded to include outpatient services and skilled nursing facility staffing and management services. Throughout this growth period, CPT has remained focused on maintaining a practice that creates values for employees so as to leverage the best possible knowledge possible.

Join Our Team at Community Physical Therapy (CPT)
Community Physical Therapy (CPT) is currently seeking a dedicated and compassionate Physical Therapist (PT) to provide weekend PRN coverage across our service areas in the greater Chicagoland and southern Wisconsin regions.

Skilled Nursing Facilities (SNFs)
Memory Care Units
Assisted and Independent Living Communities

Our leadership team is made up entirely of licensed therapists who understand the challenges and importance of therapy services and are committed to supporting our clinicians both personally and professionally.
Position Summary
The PRN Physical Therapist will be responsible for delivering high-quality, evidence-based physical therapy care. This position offers flexibility, clinical variety, and the opportunity to make a real impact in patients' lives.
Responsibilities
Provide patient-centered physical therapy evaluations and treatments
Collaborate with interdisciplinary teams to ensure optimal patient outcomes
Maintain accurate and timely documentation in accordance with CPT and regulatory guidelines
Educate patients and caregivers on home exercise programs and follow-up care
Qualifications
Graduate of an accredited Physical Therapy program
Current Illinois Physical Therapist license
SNF experience preferred, but new graduates are welcome to apply
Familiarity with Net Health software is a plus (training available)
