Responsibilities RN MDS Coordinator:
Conduct patient assessments to determine care needs. Oversee the Minimum Data Set (MDS) process and ensure timely completion. Collaborate with the healthcare team to develop and update individualized care plans. Ensure compliance with state and federal regulations. Maintain accurate patient records and care plans. Participate in quality improvement initiatives and audits. Work closely with healthcare professionals. Provide guidance and education to staff members on MDS and documentation requirements. Qualifications RN MDS Coordinator:
Current RN license in OH. Proven experience as an RN MDS Coordinator in long-term care. Knowledge of MDS processes and Medicare/Medicaid regulations. Strong organizational and communication skills. Detail-oriented with a focus on maintaining accurate records. Ability to collaborate effectively within an interdisciplinary team.
