The Telephone Triage RN works from La Clinica's Acute Care Clinic and takes a variety of patient calls including adult, pediatric, OB, medication questions, and triaging symptoms. The Telephone Triage RN plays a vital role at La Clinica, not only by ensuring patients receive timely and appropriate care, but also by providing health education and home comfort care instructions. Additionally, the Triage RN assists with transitional care management, the RN message pool and ED follow up visits.

Working closely with the primary care team, providing clinical care interventions to patients of all ages Responsibilities - chronic disease management, health education, triaging patients and calls, and serving as a valuable clinical resource to the team Qualifications:

Passion for our mission of comprehensive patient wellness Current unrestricted Oregon RN license; ADN accepted, BSN preferred One year of experience in an acute care or ambulatory care setting within the last eight years is preferred Commitment to working with diverse communities among the community, workforce and patients, including but not limited to a wide variety of race, ethnicity, ability, language, sexual orientation, gender identity and other lived and cross-cultural experiences. Epic electronic medical record experience preferred Spanish language proficiency is helpful but not required
