Cahaba Medical Care is a Community Health Center (FQHC), accredited as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance (NCQA) and Joint Commission. We have locations throughout central Alabama serving Bibb, Perry, Chilton, Shelby, Wilcox, and Jefferson Counties. Our health centers provide comprehensive primary care services, including treatment and management of chronic and acute diseases; preventative care and cancer screenings; pediatric care; women's health services; prenatal care and obstetrics; dermatological services; geriatrics; sports medicine services; mental healthcare; dental services and much more.

Cahaba Medical Care provides high-quality health services regardless of a patient's ability to pay by offering a discounted, sliding fee schedule based on income and family size. While this has been the mission of the clinic since its founding, our capabilities have continued to develop and grow through our designation as a nonprofit, Federally Qualified Health Center (FQHC). As an FQHC, we are stewards of annual federal funding that allows us to enhance our services to those without insurance and to those facing hardship - whether medical, social, financial or emotional.
About the Role:
The Pharmacy Resident at West End Pharmacy will play a crucial role in providing high-quality pharmaceutical care to patients while gaining hands-on experience in various aspects of pharmacy practice. This position is designed to enhance the resident's clinical knowledge and skills through direct patient care, medication management, and collaboration with healthcare professionals. The resident will be responsible for developing individualized medication plans and ensuring optimal therapeutic outcomes for patients. Additionally, the role involves participating in educational activities, including precepting pharmacy students and conducting presentations. Ultimately, the Pharmacy Resident will contribute to the overall mission of improving patient health and safety through effective medication use.
Minimum Qualifications: Doctor of Pharmacy (Pharm.D.) degree from an accredited institution. Current pharmacy licensure or eligibility for licensure in the state. Preferred Qualifications:
Previous pharmacy internship or residency experience. Board certification in a pharmacy specialty. Responsibilities:
Conduct comprehensive medication reviews and assessments for patients. Collaborate with healthcare teams to develop and implement patient-centered care plans. Provide drug information and education to patients and healthcare professionals. Participate in quality improvement initiatives and research projects. Precept pharmacy students and contribute to their education and training.
Skills:
The required skills for this position include strong clinical knowledge and the ability to analyze complex medication regimens, which are essential for conducting thorough medication reviews. Effective communication skills are vital for educating patients and collaborating with healthcare teams, ensuring that all parties are informed and engaged in the care process. Problem-solving skills will be utilized daily to address medication-related issues and develop tailored care plans. Additionally, organizational skills are necessary for managing multiple patient cases and participating in various projects simultaneously. Preferred skills, such as research and teaching abilities, will enhance the resident's contributions to quality improvement initiatives and the education of pharmacy students.
