Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers. With over 200 employees we strive to provide high-quality and innovative health care services to the Finger Lakes Region.

Job Summary:

The Patient Navigator is responsible for navigating health care services alongside patients who are high-risk or have barriers to care to promote better health outcomes. The position facilitates communication between patients, family members, healthcare providers, and community services.

Main Responsibilities:

Patient Risk Identification

Identify high-risk patients based off of clinical department referrals and actionsRefer high-risk patients to the appropriate Finger Lakes Community Health department, outside agency, or community resourceContact patient prior to appointment based on provider request and identified risk / patient needsSchedule transportation through outside agency for eligible high-risk patientsRefer uninsured or underinsured patients to Financial Advocacy team for health insurance enrollmentAppointment Services

Guide patient through completion of forms including:

Patient intake formsWorkers compensation formsSliding fee application

Assist patient with referral and/or follow-up appointment schedulingEducate patients on prescription medication discountsEnsure understanding of appointment information through use of printed educational materials and teach-back methodEducate patients on community and social services resources through verbal discussion and printed education materialsProvide interpretation services for patients for the duration of the patient on-site or telehealth visitAssist patients with voucher program eligibility requirements and appointment schedulingTeledental Program

Serve as bridge between the patient, Finger Lakes Community Health, and partnered teledental program including:

Patient follow-upAppointment coordinationAddressing barriers to care

Documentation

Submit telephone encounters regarding patient vouchers for processingRecord progress notes and patient education information in the electronic health record systemAddress telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and proceduresEducation and Qualifications:

High School or Equivalency DiplomaMinimum of 2 years of experience in a similar roleMust be able to fluently speak in both English and Spanish languagesExperience with interpretation or interpretation certification preferredPossesses New York State driver's licenseCurrent New York State automobile insurance in compliance with FLCH underwriting agreementPhysical Requirements:

Must be able to hear and communicate with clients and staff via phone, web media, and in person.Must be able to lift up to 30 lbsMust be able to sit for extended periods of timeMust be able to kneel, crawl, or bend into awkward spacesOSHA Level II
