We are currently hiring a Nurse Practitioner/Physician’s Assistant, to work in our Care Coordination Services. As the Nurse Practitioner/Physician’s Assistant, you would be responsible for:
Facilitate medical assessments of individuals seeking Medication Assisted Treatment
Determine availability for immediate provision of Medication Assisted Treatment
Provision of Medication Assisted Treatment
Provision of any additional medication as deemed appropriate based on prescriber assessment
Work with patient to overcome any barriers specific to the provision of medication (pharmacy access, etc.)
Work with the Bridge Clinic Coordinator and agency staff to develop and implement service plans
Participate in treatment team meetings to review and update service planning
Work with the Bridge Clinic Coordinator to facilitate referrals to community based Medication Assisted Treatment Providers for maintenance
When necessary consult with patients assigned maintenance provider to identify barriers to successful transition
Complete patient assessments and documentation through agency issued medical records systems
Complete on-going notation of any additional services provided
Work Collaboratively with the Bridge Clinic Coordinator and other agency staff necessary to ensure services are delivered in accordance with agency mission
Represent the agency in keeping with agency mission and standards of professionalism and ethics
Nurse Practitioner/Physician’s Assistant Minimum Salary: $90.00 per hour
Nurse Practitioner/Physician’s Assistant Maximum Salary: $90 per hour, w/ $200 dollar stipend
Nurse Practitioner/Physician’s Assistant Schedule: 8:30 am to 4:30 pm, Mon - Fri
16 - 24 hours per week
On call
$200 dollar flat rate for call shift
90 per additional hour of services provided while on call
Qualifications:
NP, PA, or MD
Current DEA Licensure
Current Buprenorphine DATA waiver
Current provider card in Basic Life Support (BLS)
Active Liability/Malpractice Insurance
Familiarity with and sensitivity to specialized needs of people with HIV, of people with substance abuse and mental health issues, people of color, gay, lesbian, bisexual, and transgender individuals. Ability to work with people of diverse backgrounds within client and staff populations
Background in/sensitivity to the needs of active substance users
