Psychosocial Program Coordinator (Social Worker)
for
Oncology Services.
Summary The Oncology psychosocial coordinator (social worker) is a designated member of the oncology team with the primary responsibility of coordinating the psychosocial needs of medical and radiation oncology patients and families. This position requires one to work with patients and families in a compassionate, culturally competent, and tactful manner in order to facilitate access and provide continuity of care. Works collaboratively with the service line administrator, managers, physicians and clinicians to improve outcomes for patients in our community.
Job Qualifications
Completed a course of study with specialization in clinical practices and holds a Master’s Degree from a graduate school of social work or mental health counseling; with a LCSW or LMHC license.
Required 5 years of experience in social work, preferred work in an oncology related field.
Requires strong communication skills
Demonstrated ability to work in a multidisciplinary team
Basic computer skills including basic proficiency with Microsoft Office
Experience working with community resources
Ability to work in the clinical setting
Requires strong planning and goal development
Knowledge of the impact of the disease process and of human behaviour to provide psychosocial assessments, sort term therapeutic counseling and crisis intervention to patients, families and staff.
Job Duties and Responsibilities
Collaborates with all support services and departments to safely and efficiently accomplish the departmental and service line goals and objectives, providing quality patient care
Patient evaluation and workup
Plans for continuity of care by promoting interdisciplinary patient care planning as it relates to discharges under the medical management models
Lead support groups and educational efforts
Makes suggestions for the implementation of changes to services that will enable the Center for Oncology to provide and promote the highest level of service to the community
Works with departments across the health system to promote and align oncology support services
Engages with the community to promote oncology services
Provide input on case management staff, as necessary
Ensures adherence to professional/legal standards
