OT Responsibilities:

Work cooperatively with physicians to develop treatment and care plan tailored to the client and their physicians report

Asses the home environment focusing on anything related to fall risk/intervention and overall home safety

Identify any equipment needs and develop an adaptive plan with a goal of improving client function/independence

Report client’s response to treatment or any changes in condition to DON and physician

Coordinate interventions as appropriate with other caregivers while instructing appropriate personnel, and family on intervention goals

Prepare clinical and progress summaries as well as discharge planning and submit these documents to the DON

OT Requirements:

Graduation from an Occupational Therapy program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) or predecessor organization

Successful completion of supervised fieldwork experienced as required by the educational institute where the applicant met the requirements set forth by the program

Indiana State Licensure

Abide by Occupational Therapy Code of Ethics and demonstrate knowledge about and deliver services in accordance with its standards

Two (2) years of appropriate experience as an OT

Adhere to HIPAA and maintain client confidentiality
