Summary
Under the general supervision of the Pharmacy Operations Manager, and under the direct supervision of a licensed registered pharmacist, assists in the procurement and preparation of medication for dispensing, and maintains all records in accordance with Federal and State laws and Nuvance Health policies and procedures. Participates in all pharmacy activities and works to achieve the goals of the institution and department. Job duties and responsibilities will be commensurate with experience, performance, and years of education in an accredited school of pharmacy.
Responsibilities
Maintains and complies with all state, federal and regulatory requirements for pharmacy and medication management Performs monthly quality assurance activities. Participates in shift to shift huddles and staff meetings. Completes corporate compliance and health stream modules (annually) Central Dispensing Operates pharmacy dispensing technology and troubleshoots problem/failures as appropriate Assists pharmacist in the gathering and preparing medications while adhering to policies related to unit dose packaging, regulations and proper record keeping Prepares extemporaneous compounds following established USP795 standards and practices. IV Admixture / USP 797 Compliance Prepares all IV admixtures following 797 policies and procedures Performs required daily and monthly cleaning tasks following established USP 797 standards and practices Completes routine USP 797 competencies quarterly Hazardous Drug Handling- USP 800 Performs hazardous medication preparation in compliance with USP 800 standards Completes annual didactic test on USP 800 proper practice and procedures. Inventory Management Performing technical and clerical functions associated with the ordering, receiving, packaging, and distribution of medication inventory (including controlled substances) Identifies and navigates potential drug shortages. Operates Automated Dispensing Cabinet (ADC) information system Accurately fills and maintains automated dispensing cabinets following departmental and statutory requirements. Performs required audits and maintains record keeping of controlled substance dispensing and waste Assists in preparation for state and federal inspection Medication History Obtains and clarifies patient's home medication list by utilizing drug information resources, contacting community pharmacies, interviewing patient/ family and reviewing prescription bottles Compiles and performs data entry of comprehensive medication lists including prescription, OTC, and supplement medications Clinical Tasks (under direct supervision of licensed pharmacist Interprets physician prescriptions and medication orders Provides drug information to patients, medical staff, nursing staff and ancillary department personnel Reviews patient profiles for drug interactions Medication recommendations and therapeutic drug monitoring Performs clinical interventions including dosage recommendations Assists in vaccinations Administers immunizations
Requirements
Hospital pharmacy experience preferred but not required Preferred But Not Required- Appropriate certifications as necessary for the practice setting and stage of pharmacy education (e.g. ACLS, BLS, &/or immunization)
