Coordinated Care Services Inc (CCSI) seeks a talented Mental Health Outreach and Navigation Team Liaisonto contribute their expertise in providing essential business services to our provider organizations and customers. The Mental Health Outreach and Navigation Team Liaison'serves as an engagement specialist for high-needs individuals who experience barriers to successful community living due to mental health, substance use, or criminal justice involvement' and then talk about the liaisons collaborating with community partners including healthcare and human services, social services, and law enforcement. TheMental Health Outreach and Navigation Team Liaisonworks to ensure that the system is designed to meet a person’s specific needs while engaging with them in a way that increases their desire and ability to utilize recovery services, to realize the best self-identified quality of life.
CCSI is partnering with Onondaga County Department of Children and Family Services (DCFS) and Adult Mental Health to hire aMental Health Outreach and Navigation Team Liaison.This position is employed by CCSI and working on behalf of Onondaga County Department of Children and Family Services (DCFS) and Adult Mental Health.
In this role, you will be
Serving as the first contact for high-need individuals in the community and a liaison to designated community partners and referral sources.
Processing incoming referrals, including inquiries from individuals, families, and providers, and track progress through to service connection while maintaining documentation and client records through an internal database.
Coordinating the development of system-wide treatment and support plans to include professional and natural support.
Partnering with individuals to complete an assessment of their immediate and future needs, explore supports, assist with problem-solving, and identify long and short-term solutions leading to the identification of and connection to individualized and self-defined recovery.
Conducting timely engagement activities in a variety of settings with identified individuals and their natural supports.
Establishing positive, professional relationships with adult serving systems, responding in a collaborative, culturally responsive, trauma-informed, and strength-based manner.
Other duties as assigned.
Requirements
BA/BS in Human Services with 2 years of relevant professional experience or equivalent education and work experience in the mental health, substance use, developmental disability, and/or criminal justice systems. Additional Requirements
Satisfactory completion of all required background screenings.
Full vaccination against the COVID-19 virus. Reasonable accommodations will be made for qualified religious or medical/disability needs.
Knowledge of community resources/providers.
Familiarity with mental health, medical, substance use, and criminal justice terminology.
Excellent written and verbal communication and interpersonal skills.
Experience working with individuals with disabilities.
