Ensures the timely completion of medical records for continuation of care, patient safety, coding and billing. Deficiencies for incomplete medical records are identified and tracked according to strict Joint Commission guidelines and Mount Auburn Hospital Medical Staff Bylaws and are entered into the Epic System. Analysis work queues are kept up to date within 48 -72 hours. Physicians are notified and tracked until deficiencies are determined to be complete. Daily contact with physicians is expected including answering questions on delinquent deficiencies.

Job Description:

Primary Responsibilities

Analyzes inpatient and ambulatory surgery records for deficiencies according to MAH Medical Staff Bylaws and strict Joint Commission guidelines for completion of records. Enters deficiencies into the Epic System on at least 50 – 100 patients a day depending on workload, (standard: accuracy of no more than two mistakes on physician's name and no more than eight mistakes on the deficiency identification per month). Reviews multiple tabs in Epic to determine if reports have been completed (Notes tab, Cardiology tab, Media tab, etc.). Reconciles deficiencies when marked as done or declined by the provider.

Facilitates record completion by clarifying Joint Commission guidelines and medical staff policy to physicians. Assists physicians with questions regarding their delinquencies and/or their suspension status. Reaches out to providers when Procedure Note is not completed within 24 hours of the procedure.

Reviews deficiencies for any provider who is due to receive a Suspension Letter and alerts manager when a discrepancy exists

When appropriate, prints and distributes Suspension Letters to providers according to Department Procedures/Polices.

Cross-covers Release of Information.

Performs quality checks on documents scanned into Epic.

Performs other related duties as required or requested.

Required Qualifications

High school graduate or equivalent, with some post high school training in medical office skills.

Two years’ experience in Health Information Management or Medical Office required.

One year's experience in record retrieval, scanning and indexing.

Basic medical terminology, knowledge of anatomy and physiology, and familiarity with various medical record reports in order to retrieve and communicate appropriate information from the record.

Familiarity with computers. Epic experience preferred. Must be able to use a fax machine, scanner and a photocopier.

Good English skills (verbal and written) for communication of test results, reading of medical terminology, and quick retrieval of information.

Excellent attention to detail.
