ESSENTIAL DUTIES AND FUNCTIONS:

Operational
Oversee all aspect of assigned home/s, including but not limited to, client care, documentation, cleanliness, environmental and implementation of support plans.Work with the schedulers to ensure proper staffing is in place based on the individuals’ contract.Ensure homes are personalized and reflect the interests and taste of the clients living in each home, review the home daily for health and safety concerns.Assist with implementation and have an ongoing working knowledge of all individual support plans and protocols within assigned home locations. Responsible for on sight coaching, encouraging and continuous training for Direct Support Professional (DSP) staff on the individual’s supported to ensure the highest level of support need.Assist with development and tracking of meaningful goals for each client. Monitor goal data in the home to ensure documentations is occurring at the expected frequency and communicate any needed changes to goals to the Program Coordinator.Establishing professional supportive relationships with DSP staff members to create employee engagement and a culture of appreciation.Assist in staff re-training and coaching as identified.Ensure the completion of the New Employee JET training packet and process through direct involvement, verification of skill, and sign off of documents and follow up.Complete reconciliation audits of each client cash account daily and initial form reflecting the audit was completed.Complete the Medication Box Checklist weekly and follow up with all identified concerns. Report any unresolved concerns to the Program Coordinator.Basic compliance checks are to be completed daily, weekly and as needed.Compile and review month end paperwork and turns packets into Program Coordinator.Track DSP signature acknowledgements to ensure 100% signature compliance for all support plans.Complete spend downs to include: submitting a check request, verification of all written consents for purchases have been received, carry out spend down and ensure that all spend down documentation has been filed within one week.Ensure financial limits are in compliance with the max $75 limit per individual supported at the home-site.Ensure each individual supported has a ledger and financial audit for each account ensuring that the amounts match, forms are filled out correctly, follow purchase history, and reflect deposits.Certify that property logs are kept current and accurate.Monitor documentation completion in the home location and assess for trends.Attend medical appointments with the individual supported. Ensure all appointment forms are brought to the office and scripts are faxed to the Pharmacy (as needed). Communicate the outcome of the appointment to the guardian. Ensure appointment form and health care summary is completed and filled out accurately. Communicate with Medical Coordinators to implement new protocols, ensure protocols are followed, and provide training as necessary.Schedule outings, work requirements, SOTP appointments, and doctor appointments for the individual supported. Ensure transportation arrangements are made using public transit, para-transit, staff owned vehicles and/or agency owned vehicles. Immediately report any issues securing transportation to the Program Coordinator. Communicate appointments to Program Coordinator, Guardian and Schedule Coordinator with at least 2 weeks’ notice when possible. Ensure outings and appointments do not overlap and ensure the individual supported has attended all scheduled outings/appointments.Regular communication with the guardian(s) providing updates on the individual supported progress and/or significant events (i.e. hospital visits, IRs, successful events, any change of conditions or behavioral events) and day to day events.Communicate with supporting departments (HR, Training, Nursing, Clinical, QI) as needed.Debrief with staff following significant incidents, train to and utilize Change in Condition (CIC).Monitor daily productivity and compliance of; healthy meal planning, grocery shopping, money count and cash log if applicable, documentation (electronic and in book) and cleanliness standards.Additional projects as assigned.Positive Working Environment

Complete Basic Orientation and attend other in-service training as recommended and/or required.Set and achieve performance and development goals as scheduled through the Annual Employee Assessment and Quarterly Review process.Complete and remain current in First Aid, CPR, Therapeutic Options, Infection Control and Bloodborne Pathogens Training.Safety and Health
Ensure each individuals living environment is safe and sanitary. Provide support services as needed to maintain clean, attractive homes.Ensure individuals are free from abuse, mistreatment and/or neglect and complete all necessary mandated reports.Respond effectively to emergencies and illnesses. Follow procedures and notify supervisor as required.Monitor health needs, including medications, special diets, and notify supervisor of any outstanding needs.Provide assistance to individuals to ensure they receive medications as prescribed, as required. Make consistent and accurate medication passes, following all policy and procedures for medication passes.Provide individuals with adequate supervision that is appropriate to their needs during work hours.Provide Lift and Transfer assistance (may include non-ambulatory individuals) requiring ability to bear and maneuver up to 50 or more pounds of unbalanced weight.Notify supervisor of any needed repairs or other concerns.SUPERVISORY RESPONSIBILITIES:
Is responsible for the overall direction, coordination and evaluation of staff delivering residential supported living services to specific home locations. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring and training employees; planning, budgeting, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
