Palliative Care Nurse Practitioner (APRN) - Norwalk, Connecticut
Nuvance Health, formerly Western Connecticut Health Network, is seeking a full time Palliative Care Nurse Practitioner (APRN) for palliative care services. This position will cover inpatient and outpatient at both Norwalk AND Danbury/New Milford Hospitals but will primarily be based at Norwalk Hospital. We are located in suburban Fairfield County; Danbury is just 70 miles from NYC and Norwalk 50 miles from NYC. This position works with our interdisciplinary team in a growing Palliative Care Department to provide Palliative Care Services mainly in the inpatient setting.
Norwalk Hospital has 366 beds and is also a Level II Trauma Center with approximately 50,000 ED visits per year. Established in 1885, Danbury Hospital is a not-for-profit community hospital providing Greater Danbury area residents convenient access to exceptional care. Danbury Hospital has 371 beds and is a Level II Trauma Center with approximately 70,000 ED visits per year. Both are teaching hospitals and we are associated with University of Vermont College of Medicine, Ross University, American University of the Caribbean, and Yale School of Medicine. Our Medical Group includes 900+ employed physicians and advanced practice providers, serving a geographically broad region throughout Western Connecticut and eastern New York.
This is an employed position with Nuvance Health. We offer a comprehensive compensation package, including competitive salary, generous benefits, and allowances for CMEs and Dues.
Job Summary:
The primary function of the Palliative Care APRN is to provide medical care for patients in collaboration with the physician. The APRN is responsible and accountable for the clinical management of the adult patients appropriate for palliative care and for carrying a patient caseload in collaboration with the palliative care team. The APRN is expected to assess/refer patients for both their physical and psychosocial needs of the patient. The position will report to the Nuvance Health Medical Practice System Clinical Director of Palliative Care.
Job Requirements:
* This position requires a minimum formal education of master's degree, State of CT RN and APRN license, and Federal DEA and CT Controlled Substance Registration.* At least 1 year experience as an APRN, preferably in Palliative Care, Hospice Care, and/or Oncology settings preferred, will consider new MSN graduates.* Excellent communication, clinical and interpersonal skills to facilitate patient care between hospitalists, private physicians, referral and collaboration amongst other hospital staff.* Able to work well with a large multidisciplinary team.* Able to work confidently and independently.* Board Certification in Advanced Certification in Hospice and Palliative Nursing (ACHPN) required within 2 years of hire for APRN* Willing to work Monday - Friday 8 am - 5 PM, no weekends or evenings. Willing to take limited call.

$105,000-$150,000/yr. (Annually) Salary range is commensurate with experience
