Position Summary:The Case Manager is responsible for ensuring that patient care is coordinated and managed appropriately, care and services are delivered, and for the supervision of the clinical personnel. The person must comply with accepted professional standards and principles.Job Duties:Receive case referrals and review available patient information related to the case, including facility home visits to determine hospice and/or home care needs.Assign appropriate hospice personnel to case as needed.Consult with Medical Director with any questions regarding an individual's eligibility for services.Review and evaluate each case through a variety of means to include but not limited to home visits, conferences, record review, and the services provided by clinicians.Discuss and verify impressions, instructs and guides clinicians to promote more effective performance, and delivery of quality home care services.Review patient's medical diagnosis, prognosis, medications, procedures, and clinical course.Assist clinicians in establishing individualized plan of care. .Attend case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.Willingness to assist others.Conduct monthly record reviews and communicate findings and recommendations to Patient Care Coordinator and hospice personnel.Assist Patient Care Coordinator and other supervisory hospice personnel in the planning, implementation, and evaluation of in-service and continuing education programs. Assist in formulating, revising, implementing, and evaluation organization policies, procedures, goals, objectives, all short and/or long-term.Compassionate communication with patients and their families.Assist in orientation of new employees.Market and provide community education.Must be available during normal work hours (unless previously approved by direct supervisor). Additional hours may be required to complete normal business functions and/or projects.Participation and marketing in public relations and community activities that promote the organization and its role as an effective member of the hospice and healthcare delivery system as well as customer service promotion.Assure appropriate electronic documentation.Perform other duties as requested or required, in the sole discretion of Embrace.Maintain records of nursing utilization and related activities for the QAPI program, program development, and policies and procedure review and revision.QualificationsEducation:Associates' Degree (ADN) in Nursing required.Bachelors' Degree (BSN) in Nursing preferred.Experience:Previous experience in hospice and/or related healthcare setting is preferred.Licensure/Certification/Registration:Current South Carolina (or compact state) licensure as RN (SCLLR) in good standing required.Basic Life Support (BLS) certification required.Experience:Previous experience in hospice and/or related healthcare setting preferred.Licensure/Certification/Registration:Certified Hospice and Palliative Care Administrator in good standing required. Ability to work effectively and collaboratively with nursing colleagues, physicians, and department heads.Understand principles of pain and symptom management.Understand hospice philosophy and issues of death and dying.Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.Strong organizational skills.Strong PC skills required.Strong verbal communication skills required.Must be able to work flexible hours and drive between offices, facilities, and other places,
