Apply now to see what it means to us and what it's like to work somewhere your voice is heard, your wellness is a priority, and your success matters.JOB TITLE:LPN Primary Care ManagerEMPLOYMENT TYPE:Full-time, 40 hours/weekSCHEDULED HOURS: M-F 8AM-4:30PM or 8:30AM-5PMPROGRAM/LOCATION:Behavioral Health Homes, Manchester, CTPC#:2140ABOUT THE PROGRAM: Behavioral Health Homes (BHH)BHH provides medical care coordination & health and wellness support to identified existing CHR clients with significant behavioral health and chronic health needs such as Diabetes, Obesity, Asthma/COPD, Coronary Artery Disease, Hypertension and Congestive Heart FailureABOUT THE POSITION: LPN Primary Care ManagerDuties & ResponsibilitiesIn coordination with BHH RNs, and others BHH staff assists in developing integrated behavioral health and medical treatment planning and comprehensive care management of adults and/or children with SPMI and/or chronic health conditions in CHR's Behavioral Health Homes (BHH)Provides BHH services including:Assisting BHH RNs in providing Comprehensive Care ManagementCare CoordinationHealth PromotionComprehensive Transitional CarePatient and Family SupportReferral to Community Support ServicesCollaboration and coordination with medical providers and hospitals to ensure smooth transitions of care for Behavioral Health Home patientsFacilitates health education groups.Provides training on medical diseases, treatments and medications .Works with BHH Administrative System Specialist to ensure quality data management for BHH patients in order to track health related outcomes consistent with BHH outcome goalsDemonstrates competency in age related or specialty issues and developmental needs for each population served. Works to reduce healthcare disparities whenever possibleProvides individual consultation when indicated to aid clients in understanding and managing their symptoms.Provides consultation when indicated to assist clients and families in learning about and managing mental illness, their medications and potential side effectsMay performs/ home or outreach visits and/or client transportationKeeps abreast of current ideas, theories and research regarding treatment of individuals with complex behavioral health and medical issues including factors contributing to health care disparitiesAdditional duties as required.QUALIFICATIONS:Education: Graduate of school of Nursing with LPN licensure.Experience: Experience with patients with complex medical and behavioral health issues.Licensure/Certification/Registration: Licensed Practical Nurse in State of CT. Please note that some of the listed benefits may be subject to eligibility.COMPENSATION*:$22.72 minimum - $34.08 maximum, dependent upon experience.*Actual rates are determined at the time of offer and are based off of relevant experience, as well as adherence to our internal equity policy.
