JOB PURPOSE:

Provides direct nursing care and assistance, meeting both psychological and physical needs of the residents, within the scope of educational limitation.

MAJOR RESPONSIBILITIES:

* Oversees the daily routine of the residents of The Ridgemont including dining, personal care, hygiene, activities, and social support network.

* Answers resident emergency call system promptly to respond to and assess resident condition. Contacts resident's physician and family members, if necessary, and reports all incidents to the Ridgemont Director.

* Records and tracks medications of those residents whose medications are administered. Supports and encourages those residents who are self-sufficient in medication administration.

* Observes all residents for health-related symptoms and services as a health educator when necessary.

* Documents all significant reported/observed medical related events or incidents on staff notes in resident medical files and relays pertinent information to physicians, family, and the Ridgemont Director as necessary.

* Maintains all supplies and equipment in nurse's station in a responsible and efficient manner to provide a clean and safe environment.

* Provides training, guidance, and direction to the Personal Care Aides as needed.

* Performs other duties as assigned to support the achievement of departmental and community goals and objectives.

* Seeks out and maintains ongoing positive relationships with residents by greeting residents by name, talking with residents on an informal basis, soliciting feedback regarding services and programs and responding to particular requests.

* Maintain a positive working attitude and work as a team with fellow employees.

PROBLEM-SOLVING:

The LPN provides for the daily care of residents as assigned and under the guidance of the Ridgemont Director. The LPN prioritizes workload and performs duties within Standards of Nursing Practice and individual competence and qualifications. Failure to provide quality care may inhibit resident care, decrease resident satisfaction and jeopardize resident safety. Responding to unexpected resident situations pose challenges to this position.

DECISION-MAKING:

The LPN is responsible for acting in accordance with community and nursing policies and procedures, Standards of Nursing Practice, OBRA, OSHA, and the federal and state regulations/guidelines for the daily care of residents within the Community. The LPN is required to notify the Ridgemont Director and/or physician if a resident's condition is rapidly changing. The LPN applies nursing skills in emergency situations to maintain resident stability, safety, and protect resident rights. Unusual situations are referred to the Ridgemont Director.

WORKING ENVIRONMENT:

PHYSICAL

- Mobility is necessary in order to monitor and administer resident care. Manual dexterity is used to assess resident condition, retrieve supplies, or perform prescribed treatments.

Physical effort is required, often on a daily basis, to lift, push, and pull residents while assisting with the normal activities of daily living.

Hearing within normal range is needed to determine blood pressure.

MENTAL/VISUAL

- Concentration, attention to detail, and corrected vision are needed to prepare charts, assess resident's condition, and administer treatment to resident.

ENVIRONMENTAL

- The LPN is exposed to chemicals and fumes associated with disinfectants used to clean residents rooms. Exposure to body fluids and odors relating to secretions or wastes. Possible exposure to infectious diseases.

EQUIPMENT

- Use of a stethoscope for resident assessment, blood pressure cuff to monitor vital signs, oral thermometer, physician scale, emergency call computer, washer, dryer, and whirlpool tub. Standard office equipment, including phone, fax, copier, etc.

WORKING RELATIONSHIPS:

Demonstrates concern and provides emotional support for residents and family members. As a member of the resident care nursing staff, the LPN has daily contact with residents, family members, and visitors. The LPN also has daily contact with coworkers and physicians in order to provide optimal resident care.

MINIMUM QUALIFICATIONS / REQUIREMENTS:

- Must be a graduate of an accredited school of nursing with a minimum of two years geriatric and/or hospital experience. Must possess a current LPN license in the state of West Virginia.

- Must possess and retain current CPR certification.

- Effective interpersonal communication skills in order to cooperatively interact with residents, family members, visitors, physicians, and staff.

- Must be knowledgeable of nursing and medical practices and procedures as well as laws, regulations, and guidelines that pertain to residential care.

- Must be flexible with scheduling including weekends.

- Must pass criminal background and abuse registry checks as required by state law.
