Position: Licensed Vocational Nurse (LVN)
Responsibilities:
Provide skilled nursing care to patients in their homes under the direction of a Registered Nurse (RN) or physician.
Perform patient assessments, including monitoring vital signs, medication management, and wound care.
Implement and maintain accurate patient care plans, documenting all care provided and any changes in the patient's condition.
Collaborate with interdisciplinary healthcare team members to ensure coordinated and comprehensive care.
Educate patients and their families on healthcare procedures, disease management, and medication administration.
Maintain professional standards of practice and adhere to all state and federal regulations governing home healthcare services.
Qualifications:
Current LVN license in the state of [Insert State].
Minimum of 1 year of experience as an LVN, preferably in a home health or similar setting.
