Job Summary

The nurse navigator utilizes a focused approach in following and directing inpatients with AMI to the after discharge clinic. In the process of following and directing those inpatients the AMI navigator will educate the patient/family/caregiver regarding the disease process and the necessity of close, continual monitoring to negate the need for inpatient admission/readmission. The AMI Nurse Navigator will possess the attributes of a leader/self-starter/team player enabling them to serve as a bridge between the professional staff and the patient/family/caregiver. The AMI Nurse Navigator will possess exceptional communication skills, both written and oral that will allow them to build collaborative relationships to enable the patient/family/caregiver to better navigate the health care system. The AMI Nurse Navigator will work closely with the AMI Coordinator to ensure ACC accreditation is maintained.

Responsibilities•Assumes leadership for the CAMC AMI Program. As such, drives execution by translating strategic priorities in operational reality. Drives improvement by leading through vision and values for the AMI patient population. Demonstrates the ability to lead.•Accountable for development of AMI Team goals in alignment with CAMC corporate level goals. Serves as consultant in interpretation of AMI accreditation standards and strategies to meet the intent of the standards. Maintains current knowledge of standards and stays abreast of changes from the certification body.With the Medical Director and AMI Coordinator, serves as a member of various AMI committees. Provides data necessary to determine opportunities, develop plans, and sustain success.Accountable for annual self-assessment of medical center to meet the requirement of the certification body. This assessment is based on data and collaboration with the AMI Team.Works with CHERI to identify researchable issues within the AMI Program. Responsible for oversight, coordination, and participating in resulting studies and encourage and facilitate those findings into practice. •Interface with attending and consulting physicians, residents, staff nurses, and other members of the multidisciplinary team to identify and address system processes, which impact the timely facilitation and delivery of care. Address these issues to provide optimal AMI care.Works in direct collaboration with the AMI Medical Director, Nurse Managers, AMI Clinic and hospital support of day-to-day operations, clinical management program planning, development, and marketing.Develop, coordinate, and provide community outreach activities for prevention, education and health screenings of participants. Includes development of education for CAMC patients and identification of Performance Improvement issues.•Monitors clinical outcomes and system issues related to quality care delivery, developing quality filters, audits, and case reviews, identifying trends and helping to outline remedial actions while maintaining confidentiality. Using Performance Improvement data to benchmark with state and national center. Responsible for the implementation, review, and revision of CHF standards, policies, procedures, and protocols in collaboration with the CHF Medical Director.Participate in performance improvement activities in order to share data and develop methods to optimize resource utilization in order to achieve quality outcomes.Utilize tracking methods to facilitate system changes or change in practice and participate in quality management activities, including screening, focus studies, and others. Incorporates data and information to continuously improve care and practice to enhance outcomes.•In collaboration with the interdisciplinary team, develops, maintains, and monitors AMI practice, guidelines, order sets and documentation.Make decisions affecting public relations and community interactions in collaboration with Community Affairs.•Communicate with administration, AMI Coordinator, AMI Medical Director, physicians, and Nurse Manager of acute care units and AMI Clinic regarding patient care issues.
