PRINCIPAL RESPONSIBILITIES:

Under the direct supervision of the Fr. Alfred Center Program Manager, this provides residents with social service delivery that includes case management, accessing documentation, information referrals, and individual problem solving. The Intensive Case Manager must demonstrate in their performance adherence to established policies and procedures exhibiting the defined characteristics associated with attendance and punctuality.

The Intensive Case Manager will create the curriculum of group independent living skill sessions. The Intensive Case Manager will function as part of the substance abuse treatment team at the Fr. Alfred Center. The individual will demonstrate skills and ability in promoting excellent patient care in a patient centered residential substance abuse treatment setting with integrated primary care and behavioral health.

MAJOR DUTIES:

Support individuals on a caseload in accessing community-based resources related to housing, education, employment, signing up for insurance, accessing personal documents, etc.Knowledgeable in assessing client acuity, crisis intervention, and suicide and crisis management.Collaborates with other St. Anthony programs providing services to the residents (the Dining Room, the Tenderloin Tech Lab, the Medical Clinic, Workforce Development and the Social Work Center) and with outside community agencies.Provides individual counseling and case management for a regular caseloadDeveloping and administering group sessions related to substance abuse and/or independent living skillsCollaborate with the team to develop treatment plans, and document services appropriately.Collaborate with client's physician and individual therapist as appropriate.Exchange information (within legal parameters) with other health and/or rehabilitative agencies to provide continuity of care for clients receiving services from more than one agency.Facilitates case review meetings and participates in staff meetings, agency meetings and other meetings as assigned.Assist in planning, developing, and implementing case management procedures and other related guidelines and policies.Conduct initial screening, intakes, needs assessment, and service plans.Documents client benchmarks and other required information into Efforts to Outcomes (ETO), daily as required by the program (case notes, treatment plans) in a timely manner.Assist clients in filing for benefits and other eligible services.Provide Payee services to ongoing guests.Complete Rental Assistance applications as needed.Provide direct services in an equitable manner and be good stewards of the organization's charity grant resources.Attend case conferences regarding guests, or arrange same as needed (to include other St Anthony Foundation (SAF) work units and/or outside agencies).Engage in creative problem solving to ensure residents' needs are optimally met.Adhere to the guidelines of the St. Anthony Foundation Safety Program.Note significant events in the staff log.Fills out medication sheets and all other required paperwork completely and accurately.Participates in required program staff meetings and St. Anthony meetings.Assist in training and instructing new social workers, interns and volunteers.Other duties as assigned by the Senior Program Manager or ManagerSkills & RequirementsMINIMUM QUALIFICATIONS:BA or equivalent experience desired. MSW preferred but not required.Three or more years of demonstrated experience in social service work, preferably with diverse co-occurring populations, persons with history of homelessness, mental health, substance abuse, history of criminal involvement, HIV/AIDS, etc. Case management background essential.Two years of current experience in client caseload management and group counselingAbility to work in a substance abuse environment, effectively and comfortably work as a member of an interdisciplinary team.Ability to work effectively with diverse residents, volunteers and other staff.Ability to set appropriate limits and behavioral boundaries.Ability to work autonomously as well as cohesively with a team.Knowledge of SF/Bay Area social service resources or resourcefulness to acquire same is desired.Proficient in Windows, MS Office.Demonstrated ability to exercise sound independent judgment and effectively manage time and resources.Strong written and verbal communication skills desired.Familiarity with the use electronic medical record software.Knowledge of applicable mental health laws and ethical standards.Preferably: Experienced in outpatient and residential substance abuse treatment as well as detox protocols and services.Fluency in Spanish desired. Conversational in Cantonese and Vietnamese also desired.CPR and First Aid certification within 90 days of hire.Valid CA Driver's license with excellent driving record.Commitment to the St. Anthony Foundation mission and Statement of Values and a desire to work for a social service agency serving the poor.
