Advanced Practice Provider (APP)
Responsibilities: As an APP, you will play a crucial role in our Rural Health Care clinic, ensuring the delivery of high-quality healthcare services to our diverse patient population. Your responsibilities will include:
- Conducting thorough patient assessments and developing personalized treatment plans.
- Collaborating with the healthcare team to deliver comprehensive care and excellent patient outcomes.
- Providing patient education and counseling to promote wellness and disease prevention.
- Embracing the values of compassion, respect, and empathy to foster a healing and supportive environment.
Core Duties:
- Comprehensive Patient Care: Provide team-based integrated and culturally respectful patient-centered care to a designated panel of patients in accordance with medical best practices and organizational policies and procedures.
- Clinical Assessment and Diagnosis: Conduct thorough patient history taking, perform physical examinations, and make accurate diagnoses. Develop and implement effective treatment plans and perform procedures within the scope of licensure, approved credentials, and clinical best practice standards.
- Diagnostic Testing and Follow-up: Order appropriate diagnostic tests based on clinical findings and best practice standards. Interpret test results and ensure timely follow-up with patients and their families as needed.
- Electronic Medical Records (EMR) Management: Complete accurate coding and update EMR documentation for each patient encounter following clinic procedures. Record physical findings, treatment plans, and prognoses based on patients' conditions.
- Collaboration and Care Planning: Discuss cases with physicians and other healthcare professionals to develop comprehensive patient care plans. Submit healthcare plans and individual patient goals for periodic review and evaluation by the Medical Director.
- Medication Management: Prescribe new medications and refills, as well as other treatment modalities such as physical therapy and inhalation therapy, adhering to best practice guidelines.
- Patient Education and Counseling: Engage patients and families in individual and family-focused care and behavioral change. Provide education on health risk factor reduction, treatment compliance, and wellness promotion. Offer age-appropriate anticipatory guidance and counseling on various health topics.
- Quality Improvement: Actively participate in clinic and operational quality improvement processes aimed at achieving targeted clinical outcomes. Contribute to the development and review of clinic policies and procedures.
- Team Collaboration: Attend scheduled team, provider, and staff meetings to foster effective communication and collaboration within the healthcare team.
- Professional Development: Stay informed about the latest medical advancements and trends. Seek opportunities for ongoing education and professional growth.
- Other Duties: Perform any other related duties assigned or requested in a professional and efficient manner.

Qualifications: To be successful in this role, you must possess the following qualifications:
- Current and valid licensure as an Advanced Practice Provider (FNP/PA) in the state of California.
- Strong clinical acumen and the ability to make critical decisions independently.
- Excellent communication skills, fostering effective patient interactions and seamless collaboration with the healthcare team.
Minimum Qualifications:
- Academic degree required.
- Completion of a specialized Physician Assistant Training Program/Family Nurse Practitioner Training Program required.
- Successful completion of a national examination prior to being licensed by the Physician Assistant Board/Nurse Practitioner-Board.
- Licensed as a PA or FNP in good standing from the California State Board.
- National Provider Identifier (NPI) member, with a satisfactory report from the National Practitioner Data Bank.
- Ability to successfully meet Dignity Health's credentialing program and privileging processes upon hire and biannually thereafter.
- One (1) year of experience as a Physician Assistant in an outpatient primary care clinic or community health center required.
- Sufficient training to maintain prerequisite certifications required.
- Current Basic Life Support (BLS) certification through the American Heart Association (AHA) is required.
Preferred Qualifications: (Include any additional qualifications that are not strictly required but would be considered advantageous for the role)
- Experience working in a rural healthcare setting.
- Specialization or certifications in relevant medical specialties.
- Demonstrated commitment to serving diverse patient populations.
- Proficiency in electronic health record (EHR) systems.
