GENERAL SUMMARY

Gaylord Primary Care practice seeking an experienced physician!  Currently we have one immediate opening. Physicians interested in this position should be excited and willing to be a part of a practice building culture, developing a team-based approach to care, and eager to mentor new graduates, residents, and advanced practice providers.  Physicians interested in this practice should be passionate about high quality care and service with a focus on improving the wellbeing of all their patients.  As part of the Munson Healthcare system, there is opportunity to have medical students and residents rotating with you.

Gaylord Primary Care is an established, busy ambulatory clinic located on the campus of Munson Healthcare Otsego Memorial Hospital.

STANDARDS OF CARE

  1. Manage chronic conditions and acute episodic illnesses and minor injuries, using best practices, i.e., evidence-based practice
  2. Obtain relevant health and medical histories from patients and/or caregivers
  3. Perform physical exams appropriate for age and history
  4. Identify medical and health risks and needs
  5. Order, conduct and interpret appropriate diagnostic tests
  6. Formulate the appropriate plan of care based on history, physical exam, clinical findings and any diagnostic findings
  7. Develop care plans and communicate plan to patient, their care giver, or family
  8. Perform procedures within the scope of expertise (e.g., I&Ds, suturing wound closure, etc.)
  9. Identify non-pharmacologic interventions and prescribe non-pharmacologic therapies; identify pharmacologic interventions and prescribe pharmacologic therapies
  10. Provide relevant patient education
  11. Make appropriate referrals to physicians and other health professionals and community agencies
  12. Participate in quality improvement review on a periodic basis, including systematic review of records and treatment plans, helps disseminate findings and implement plans for improvement
  13. Serve as an educational resource for ancillary staff
  14. Complete and maintain written electronic records in a timely fashion and in compliance with MHC standards

Reports any unsafe situation/equipment according to hospital procedure.
  • As a health care provider, primary responsibility of the physician is to promote and maintain health in all age populations; and to prevent disease and illness exacerbation.
  • Assists in policy and procedure development for OMH Medical Group operations and clinical interventions.
  • Maintains visibility within the community as a primary health care provider. Serves as a role model to clinic staff, faculty, and the community at large.
  • Performs other duties and responsibilities as assigned.

STANDARD QUALIFICATIONS

Education / Training: 

  • MD or DO
  • Successful completion of an approved U.S. Residency training program approved by the Accrediting Council for Graduate Medical Education (ACGME) of the American Medical Association, or the American Osteopathic Association or its equivalent.

Certification, Licensure, Registration:

  • Board eligible/certified for internal or family medicine practice in the State of Michigan

OTHER QUALIFICATIONS

  • Candidate must apply for and be granted applicable clinical privileges through MHCs credentialing processes before rendering services
  • Good judgment and problem solving skills.
  • Excellent written and verbal communications skills.
  • Ability to make quality, independent medical decisions.
  • Time management and organizational skills.
