The Night Ministry is a Chicago based organization whose mission is to provide human connection, housing support, and health care to members of our community who are unhoused or experiencing poverty. We've successfully provided these lifesaving services for 45 plus years, everyday providing a lifeline to our most vulnerable neighbors. As the CTA Outreach Case Manager you will w ork with the CTA outreach team and provide case management services to clients. Develop and maintain quality referral relationships with social service agencies. Provide follow-up for individuals referred by Outreach and Health Ministry staff. Assist program participants, many of whom are experiencing homelessness or are at risk of homelessness and are medically vulnerable, to achieve housing and health stability.
Responsibilities:
Provide Case Management to CTA Outreach Program clients, offering on-site services and follow-up support.
Develop and maintain relationships with referral resources covering key needs of OHM participants, including housing, health, mental health, spiritual care, and addiction services.
Ensure accurate and timely data entry in external and internal databases, including EHR and ETO.
Occasionally accompany Outreach staff on Street Medicine Outreach and the Health Outreach Bus, working varying shifts including evenings, days, and weekends.
Collaborate closely with staff and volunteers, driving the Outreach Van as scheduled.
Offer compassionate support and referrals, employing motivational interviewing and harm reduction methods to engage visitors in services.
Demonstrate competency in trauma, LGBTQQIA, and street culture, working effectively with diverse populations.
Attend required meetings and represent the program and agency to congregations and other groups.
Participate in agency public relations and fundraising activities as assigned.
Perform other assigned duties.
Qualifications:
Bachelor's degree in social service or related field, or equivalent work experience required.
Two or more years of outreach, health, or case management experience.
Highly organized and detail-oriented with strong interpersonal skills and professional boundaries.
Must possess a driver's license with a good record and access to a car, willing to travel locally.
Ability to remain calm in emergencies, assess situations quickly, and respond according to Night Ministry policies.
Comfortable working with diverse populations, including unhoused individuals, sex workers, youth, and LGBTQ+ communities.
Familiarity with referral sources preferred.
Spanish language proficiency encouraged.
