Reason for coverage: Loss of provider, we have patient panelsSchedule: Our Clinic is open for Provider schedules 830 7 M,T, H, F, on Wednesdays, and 830 2 on SaturdayShift: We are flexible within the above time frames.Call: NoPatient volume: For a full day, around 17Scope: 100% Pediatrics EMR: Next GenHospital: Referrals to Salem Health, Silverton (Legacy) or Stayton (Santiam) hospitals depending on where a patient lives.Group/support: We have a cadre of nurses, medical assistants, and other licensed medical providers on siteRequirements (boards, license): Licensed to practice in Oregon as MD or DO with DEA designation.
