This position offers a unique opportunity for an active and experienced Licensed Clinical Social Worker (LCSW) interested in patient care and administrative responsibilities with our value-based programs. This position will collaborate with department physician and operations leadership to care for our underserved patient population while assisting with optimizing our value-based programs.
GENERAL SUMMARY
Provides direct care and psychiatric social work assistance to patients. Investigates case situations and presents information to other members of the health team. Interprets psychiatric/behavioral treatment to patients and patient's family. Refers patients or patient's family to other community resources as needed; works in conjunction with other health care team members to coordinate care; participates in clinical research; and other administrative duties as assigned.
ESSENTIAL FUNCTIONS
Maryland Primary Care Program (MDPCP)/ Value Based Programs (30%)
Serves as the lead coordinator for the MDPCP program for administrative and case management requirements to include other value-based program as assignedCoordinates with MDPCP case management team and enables delivery and outreach of case management services.Coordinates with physician and operations leadership with developing and implementing strategies to improve quality metrics and increase incentive earnings for various MDPCP and other value-based programs.Utilized data and resources to develop, implement, and facilitate various practice-wide quality-improvement efforts.PATIENT CARE/CLINICAL (60%)
Develops and maintains Department's Behavioral Health program, drafting supporting policies, procedures, and guidelines for the program.Assesses patients' psychosocial status. Communicates with physicians to report patient progress, needs, concerns, and problems. Works closely with all members of the health care team's psychosocial plan of treatment, including discharge plans, based on the psychosocial assessment and accepted social work interventions.Maintains up-to-date knowledge of field by participating in continuing education activities.Acts as a resource person for other members of the health care team, providing in-service education and consultation in understanding behavioral health matters.Maintains up to date- knowledge of Medicare, Medicaid and other third party payor policiesMaintains accurate and timely records, including clinical and progress notes, according to department policy. Adheres to all department/hospital policies. Uses the electronic medical record in accordance with the department and/or FPI standard operating procedures and processes.Provides positive customer relations in dealing with patients, families, physicians, fellow department employees, other hospital departments, referral resources, services agencies, and others.EDUCATION and/or EXPERIENCE
Master's degree or PhD in Clinical Psychology.Minimum of two years clinical experience in an academic setting with curriculum development preferred.Licensed Clinical Social Worker (LCSW) certification is required.
