Seeking Post Acute Care Physician

This outstanding Full-time opportunity will provide patient care within a high-volume; quality focused and established Post-Acute ACO Model of Care

• Will work in Skilled Nursing Facility to provide seamless transitions in care and improved quality outcomes
• Interest in working in post acute program in integrated, coordinated and collaborative environment
• Physician supported by Advanced Practice Practitioner
• Board Certified MD/DO in Family Medicine / Internal Medicine with Specialty in Geriatrics preferred
• Experience in Long Term Care and Acute Care
• Experience as Hospitalist and/or SNFist is a plus
• MA License
