MDS Coordinator LPN LVN

Position Summary: The MDS Nurse (LPN/LVN), under the direction of the Director of Nursing (DON) is responsible for coordinating the Resident Assessment Instrument (RAI) process including completion of an accurate Minimum Data Set (MDS) and collection of data to develop the Interdisciplinary Plan of Care.
