Owner and/or Director of OperationsWhat You Will Do (Responsibilities)The RN Care Manager will be responsible for client care and caregiver supervision and will be expected to evaluate clients’ needs, monitor aides, perform nursing assessments,
prepare care plans, and ensure assigned caregivers provide appropriate care. Additionally, the manager will also be responsible for evaluating caregivers’ skills and providing ongoing training and support.Job DutiesTravel to clients' homes and complete all documentation for nursing assessments and observations.Perform regularly scheduled evaluations pertaining to clients and caregivers.Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan.Coordinate with management on implementing and revising operational procedures related to quality client care.Utilize professional assessment skills to identify the physical, mental, and psychosocial needs of clients and be able to develop a care plan to meet these needs in a multi-disciplinary approach.Function as a resource and mentor to peers and subordinates to maintain professional nursing standards.Act on the client’s behalf to ensure patient rights.Strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitudes and practices displayed by other staff members.Document and respond to any client complaints concerning a caregiver and review complaints with the management team.Coach all staff in proper safety procedures and consistently reinforce procedures.Complete all documentation for initial assessments, observations, and planning evaluation pertaining to clients.Provide supervision and consultation of in-home caregivers.Oversees skills assessment performed by in-home caregiver to ensure competency (skills verification)Documentation that in-home caregivers are competent to perform client care tasks or activities.Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan.Utilize professional assessment skills to identify the physical, mental, and psychosocial needs of the client and be able to develop a care plan to meet these needs in a multi-disciplinary approach.Function as a resource and mentor to peers and subordinates to maintain and uphold professional nursing standards.Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitudes and practices displayed by other staff members.Able to read and interpret and stay abreast of Virginia home care state licensure regulations.Perform all other duties as assigned.QualificationsAble to work as a team memberAble to maintain confidential informationAble to communicate effectively with company staff, the clients, and client’s familiesAble to always handle occasional physical and verbal mistreatment from clients in an appropriate professional mannerAble to supervise (instruct, assign work, etc.) caregivers while on dutyExcellent verbal and communication skills, adaptable in different situations, and possess excellent client interaction skillsAble to multi-task and work independentlyBenefitsFlexible schedule and payMileage reimbursementCell phone reimbursement (If applicable)Company laptop (If applicable)Advantage core and health plan optionsDental, vision, and health insurance options (full-time 30+ hrs )Discount plan, critical illness, telehealth, perks and benefitsEducation and ExperienceCurrent RN licensureADN, bachelor’s (Preferred)1 year of training and experience in direct health care service delivery.Minimum of 1 year within the last 5 years of supervisory/administrative management experience in health care.Homecare assisted living or geriatric experience preferred.Valid Driver's License and reliable transportationThis location is independently owned and operated.
