Job Description
JOB SUMMARY:
Under the direction of the Chairman of Department of Medicine, Chief Medical Officer for Gotham Health and Medical Director of Gotham Health North Brooklyn, provides comprehensive adult primary care(preventive and sick care) services for adult patients of local communities. Coordinates patients' specialty care referrals utilizing all the resources when needed and follow them up. Ensure patient/ provider continuity and access to timely and appropriate levels of care.
RESPONSIBILITIES:1. The duties and responsibilities for this group of full-time providers are partially dictated by the New York State Medicaid Managed Care Program (NYS MMCP) requirements.2. Carries and manages their assigned panel of adult patients providing comprehensive primary care across various levels of service. Where required by the practice, responsible for the supervision of mid-level providers.3. Obtains history, performs Physical Exam (PE) and assesses medical and social determinants of health and works with care team to give guidance to patients4. Orders, performs and interprets appropriate screening tests and formulates diagnoses.5. Directs and/ or performs treatment plan including administration of medications and immunizations.6. Documents all patient care accurately in the electronic health record in a timely manner. Closes and signs charts electronically within 24 hours of each patient visit7. Coordinates referrals to specialty clinics and follows up to ensure proper care of patients with acute and chronic problems.8. Performs regular chart reviews to ensure that medical care of patient is current.9. Acts as a consultant to staff, provides medical leadership to clinic staff and actively participates in daily pre-clinic meetings or "Huddles."10. Provides a scope of care and clinic participation in accordance with the New York State Patient Centered Medical Home (PCMH) Standards and Guidelines11. Participate in the H+H Antibiotic Stewardship program by:
a. Following prescribing guidelines
b. Educating patients and family members about the risks of antibiotic overuse
c. Help provide education to non-clinical staff on this subject12. Maintain a work schedule, including evenings and weekends that ensures availability and continuity of care.13. Maintains a satisfactory workload that is consistent with corporate minimum standards.14. Directs participation in 24-hour phone coverage for patients enrolled in all off-site medical centers.15. Must meet all compliance requirements (health and safety, infection control, patient confidentiality, sexual harassment, domestic violence, and customer training, etc.).16. Maintains medical staff privileges at New York City Health and Hospitals/ Woodhull and Gotham Health and updates all required components including CPR certificate, required CME certificates, etc.17. Performs any and all other related duties as assigned by supervisory staff18. May provide clinical supervision/precepting to resident's physicians, nurse practitioners and nurse practitioner students, and medical students19. On Call once a month for critical Lab results

Minimum Qualifications1. Graduation from an approved Medical School; and,2. Licensed to practice in New York State; and,3. Completion of an approved residency in the specialty and Board Certified or Eligible (if Eligible, must obtain certification within five years of eligibility); and,4. Must meet the criteria for credentialing and privileging in the department as defined by the Medical Staff By-Laws of both the facility employing the Physician and the back-up hospital, if these are different.5. Must meet all NYS Medicaid Managed Care requirements.

QUALIFICATIONS:1. Medical Doctor (MD) degree from approved medical school, and2. License to practice medicine in New York State, and3. Completion of an approved residency in the specialty and Board Certified or Board Eligible (if board eligible, must obtain certification with five (5) years of eligibility), and4. Must meet the criteria for credentialing and privileging in the Department of Medicine as defined by the Medical Staff Bylaws of both the Gotham Health Care and Woodhull Medical and Mental Health Center5. Must meet all New York State Medicaid Managed Care requirements.
