Title: RN-Care Manager Location: Midtown Org Unit: Population Health Management Work Days: Exemption Status: ExemptSalary Range: $98,100.00 - $108,600.00 *As required under NYC Human Rights Law Int 1208-2018 - Salary range for this role when Hired for NYC Offices Position SummaryUses clinical expertise and techniques to identify and manage patients in need of care management. Adopts processes and workflow to achieve improved quality, satisfaction and safety of care, improved health of populations and reduction in costs.Job ResponsibilitiesConnects patients and caregivers to internal and external health care and social services.Develops detailed care plans for patients based on assessment of patient's condition, needs, social support and barriers.Documents in patients' medical records and other data systems to ensure effective data collection and evaluation.Increases overall compliance with medical appointments and routine screenings and improve medication adherence.Provides education on disease processes, medications and self-management methods.Provides ongoing support to improve clinical outcomes and reduce ER visits and hospital re-admissions.EducationAssociate's Degree in related fieldExperienceNew York State license to practice as a Registered Nurse, Social Worker or Nurse Practitioner.Experience with principles of Care Management and Care Coordination.Experience with Patient Centered Medical Home requirements and processes preferred.Experience with evidence based guidelines for the management of chronic conditions preferred.Bachelor's degree preferred.Knowledge, Skills and AbilitiesAnalytic skills and data management.Flexibility, tolerance and demonstrated ability to take initiative and to prioritize multiple tasks with attention to details.Experienced user of an Electronic Medical Record, EPIC preferred.Function at a fast pace and able to interact with a wide variety of people and situations while maintaining a customer focus.Excellent communication skills.Aware of scope of practice boundaries, comfortable seeking direction and assistance from appropriate resources.Proficient in Microsoft Office Suite.Excellent organization skills with customer service delivery in a way that patients are willing to accept.Licenses and CertificationsBLS Certification (issued by the American Heart Association)Infection Control CertificateWorking Conditions/Physical DemandsWeill Cornell Medicine is a comprehensive academic medical center that is committed to excellence in patient care, scientific discovery, and the education of future physicians and scientists in New York City and around the world.
