TheRegistered Nurse provides direct nursing care in accordance with establishedpolicies, procedures and protocols of the organization. The nurse maintains theknowledge and skills necessary to provide care appropriate to the age of thepatients served on the assigned unit. The nurse facilitates the coordination ofpatient care within the interdisciplinary care team and provides patienteducation to assure safe patient discharge.u5:p

Performs daily rounds on all patients assuring qualitycustomer service by coordinating and taking action on any requirements notmet.u5:p

Develops a plan of care based on assessment thatreflects the patient's physical, psychological, social, educational,developmental and rehabilitative needs based on patient's age, ethnicorigin, economic status, lifestyle, creed or philosophical beliefs.u5:p

Assesses the patient's condition and nursing needs,sets goals, and prescribes appropriate nursing actions to meet those goalsand the physical, psychological, social, and rehabilitative needs of thepatient, including discharge planning.u5:p

Assesses, intervenes, and reassesses the patient'sself-report/symptoms of pain promptly and takes appropriate actionaccording to standard operating procedure.u5:p

Identifies the early warning signs of a change in apatient's condition and responds appropriately to a deteriorating patient,including contacting responsible clinicians as indicated by standardoperating procedure. Assists with or institutes emergency measures forsudden adverse developments in patient conditions.u5:p

Reports pertinent observations and reactions regardingpatients to the appropriate multidisciplinary team member and recordsthose observations accurately and concisely. Ensures that intervention orreferrals are made promptly. Completes and submits all incident reports(patient, visitor, staff) prior to the completion of each shift.u5:p

Establishes and maintains communication and utilizeseducational opportunities with patients and other significant individuals.u5:p

As early as possible, begins discharge planning,appropriate education and counseling based on identified needs to optimizepatient/family understanding. Evaluates and documents according tostandard operating procedure.u5:p

Assigns or delegates, within the scope of practice,education and experience, the responsibility for performance of nursinginterventions to other persons for which that person has the necessaryskills and competence to accomplish safely.u5:p
