Position Overview
The
Hampton Roads Community Health Center
is looking for physicians to join the medical team. We have five (5) community health centers in the Hampton Roads area of Virginia. Hampton Roads is a metropolitan area in southeastern Virginia that includes the cities of Chesapeake, Hampton, Newport News, Poquoson, Suffolk, Virginia Beach, and Williamsburg. It also includes the counties of Gloucester, Isle of Wight, James City, Mathews, and York, and Currituck County, North Carolina. The Hampton Roads region is known for its connection to water, and includes the Chesapeake Bay, the Atlantic Ocean, and many rivers. It's also home to one of the world's largest natural harbors, and is the northernmost major East Coast port of the United States that's ice-free year-round.Under limited supervision, this exempt position works as part of the medical provider team providing medical services to the patients of the community health center. Reports to the Chief Medical Officer (CMO).
Key responsibilities include:
Essential Functions
Provide primary medical care by caring for all patients in accordance with the physician's medical specialty; develops a plan of care for each patient, including complete medical history, physician examination, diagnosis, appropriate treatment and/or referral, including hospitalization, when necessary; stresses the importance of and keeps in compliance with clinical guidelines relating to preventive health care measures; and uses all available resources in diagnosis and treatment, such as laboratory and radiological testing, in an appropriate and cost-efficient manner.
Confer with consulting physicians, nurses, patients, and patient's families concerning treatment and care of patients; refers those cases, which require specialist services, but maintains responsibility, assuring that continuity of care is provided; serves as a clinical preceptor relating to medical education; educates support staff where needed to maintain patient safety and workflow; completes electronic health records according to practice standards; and closes charts within the appropriate time frame.
Participate in the development and implementation of a regularly up-dated performance improvement plan for the consortium and disease collaborative(s); participates in the Health Promotion/Disease Prevention activities required by the U.S. Public Health Services; and participates in the health center's quality assurance program, including attending morning huddles to help maintain Patient Centered Medical Home recognition and Joint Commission Accreditation.
Perform administrative support work such as word processing, creating spreadsheets, data entry and retrieval; orders, monitors and distributes program supplies/resources; and other information to ensure accuracy and conformance to established procedures and policies; and counsels patients regarding patient responsibilities and rights; adheres to patient confidentiality guidelines.
Perform other duties as assigned.
Required Qualifications:
Graduation from an accredited medical school with a degree of Doctor of Medicine or Osteopathy. Completion of an approved residency program in their field. License to practice medicine in the Commonwealth of Virginia. Board certified in their field. Must be certified in Basic Life Support techniques.
Requires an acceptable background check to include state and local criminal check; satisfactory results from a pre-employment medical evaluation and substance abuse testing and is subject to random alcohol and controlled substance testing.
