40 hours, Monday - Friday 7am-3pm
providing case management, advocacy, educational, and supportive services in a program serving men with major mental illness who require intensive supports to reside in a community setting. The individual in this role carries out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge. per hour dependent on level of education.Responsibilities:Providing clinical knowledge regarding severe and prolonged mental health and substance abuse disorders through training and educationSupporting the Director/Manager in leading an effective program, including supervising shift activities, reviewing, and implementing the daily schedule, and providing clinical leadershipWorking with the treatment team to manage clients’ mental health and assessing for risks, implementing risk management plans as necessaryProviding recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supportsProviding support, skills training and education in areas including daily living skills, nutrition, financial management and symptom managementMonitoring positive outcome measures and ensuring program compliance through accurate and timely data reportingCompleting assessments, treatment plans, and other documentation electronically and on paper in a timely mannerActing with compassion, integrity, and ethicsGilead employees are held to standards and regulations set by the Department of Mental Health and Addiction Services (DMHAS) and other regulatory bodies. These standards are outlined in Gilead’s “Standards of Conduct” policy.
