A health services organization in New York City is looking to add licensed Registered Nurse (RN) to their team in Brooklyn as a Case Manager. In this role, the Case Manager (RN) will be responsible for developing a plan of care and coordinating the delivery of healthcare services for persons receiving services in an assigned program unit.

9am to 5pm; 8am to 4pm; or, 7am to 3pm (1/2 hr unpaid lunch)Responsibilities:
The Case Manager (RN) will:
Coordinate the delivery of routine and specialty healthcare services to address assessed needs of persons receiving services in assigned program unitAssess and establish healthcare outcomes for persons receiving servicesDevelop plan of nursing services and oversee the implementation of the healthcare plan by educating person's receiving services, DSP/Caregivers and families in the plan of nursing servicesEducate, monitor and supervise staff in carrying out ordered treatment plan related to health needsAssign the Direct Care staff to perform health care treatments/proceduresEvaluate the effectiveness of preventive/ongoing health care and makes recommendations/ referrals for alternative care, as necessary.Act as a liaison to communicate health information between and among team members, families, other agencies, hospitals, clinics and all other service providersComplete and maintain all required written documentation on the approved electronic health record systemConduct general and individual specific health related trainings for persons receiving services, direct care staff, and other team members as neededProvide on call triage coverage via cell phonePerform other duties, as neededQualifications:
Graduate of an accredited School of NursingNYS Registered Nurse (RN) licenseSolid assessment, clinical, and documentation skillsPatient orientedGreat interpersonal skillsExcellent communication skills (written and verbal)Strong attention to detailHighly organizedDesired Skills:
Bachelor's Degree in NursingExperience working with the Developmental Disabilities population
