We are seeking employees who are excited by the idea of lifelong growth in the skills they bring to helping those in need.Program DescriptionServing adults living with serious mental illness, the Adult ICM team provides an intensive array of recovery-oriented services and supports, including linkage to housing, basic needs, physical health care, benefits assistance, and employment services, and integrated mental health and substance abuse treatment services. Our multidisciplinary team consists of clinical case managers, a psychiatric medical team, client support specialists, administration, and clinical supervisors.Job DescriptionThe Clinical Case Manager evaluates the needs of clients who are living with mental, emotional, and/or substance abuse problems and develops a treatment plan in coordination with providers, family members, and support systems. Treatment plan typically includes coping skills development, psychiatric medication management, and linkage to basic needs. Clinical Case Manager helps clients in applying for and obtaining other social services while providing a safe and supportive environment for agency’s clientele and adhering to laws regarding confidentiality and reporting requirements. Services are provided both in the field and in the office and include individual and group therapy, crisis intervention, case management, client advocacy, and education. The case manager must maintain accurate detailed observation and progress records for electronic billing data entry. Referrals for the Adult ICM caseload come from San Francisco Behavioral Health Services and the caseload is about 17 clients. Please note that this position requires both in the field and in the office work with clients.Program Specific ResponsibilitiesProvide case management, mental health services, and crisis intervention for adults living with severe mental illnessDevelop and maintain good clinical relationships with clientsMust be able and willing to outreach to clients in all San Francisco neighborhoods and meet clients in various environments, including client’s homes, SROs, and Board and CaresAssist clients to maintain stability in the community by offering support and linkage to needed servicesCollaborate with community partners in housingProvide case management and independent living skills education to clients in the office and in the homeResponsible for 15 hours of direct (face-to-face) service to clients per weekMaintain timely documentation of services providedCarry a caseload of up to 17 clientsProvide officer-of-the-day responsibilities as part of Adult ACM teamMinimum Qualifications:Masters degree in Social Work, Counseling, Marriage and Family Therapy or Clinical Psychology with a current California State registry certificationExperience supporting a mental health program a plusExperience providing case management and mental health services to those living with a severe mental illness and/or who are dually diagnosed a plusExperience with Medi-Cal billing/documentationComprehensive range of mental health and substance abuse counseling, assessment, and crisis intervention skillsWorking knowledge of word processing (i.e. Microsoft Word, Excel and Access)Willingness to work from a strength-based, harm reduction, client-centered model with an emphasis on evidence-based practicesAbility to work cooperatively on a multi-disciplinary teamStrong organizational, communication, time management, and interpersonal skillsAbility to maintain accurate and timely documentation of clinical workAbility to thrive in a fast paced environmentValid CA Driver’s License preferredMust be a team playerAdditional Information:Division: AdultPosition: 100% FTEReports to: Program DirectorSEND RESUMES TO: "jobs@felton.org"
