Dartmouth Health Manchester is currently seeking an experienced Family Medicine APRN or PA to work with our Family Medicine team to manage the health and wellness of our patients. As part of a Medical Home, you will work collaboratively to provide comprehensive care to our patients in Southern New Hampshire. Working under the general direction of the medical director, you will practice autonomously as well as in collaboration with physicians, APRNs, and PA colleagues and other healthcare providers to provide primary healthcare to patients in our practice.

Preferred Qualifications:

3+ years working in a primary care clinical settingPrior experience managing a primary care panel of patientsDartmouth Health Manchester Family Medicine works within the community to ensure that the patients of our community have access to the best possible care. The practice offers a collegial, supportive and learning environment. There is direct access to DH specialists to support complex care as needed.
