Job Summary

We are seeking a Designated Coordinator to join our team. In this role, you will work collaboratively with clients to determine their medical needs, develop the best course of action, and oversee their treatment plans, ensuring each client gets high-quality, individualized care. The ideal candidate is compassionate, patient, and knowledgeable about Home Based Community Services and PCA Services.

Responsibilities

Oversight of Service Plans: Manage and oversee Heavens Home Health Care Corporation's responsibilities assigned in the coordinated service and support plans and addendums.

Facilitate Outcomes: Take necessary actions to achieve desired outcomes according to section 245D.07 requirements.

Staff Instruction and Assistance: Provide guidance and support to direct support staff in implementing coordinated service and support plans and achieving service outcomes, including direct observation to assess staff competency.

Service Delivery Evaluation: Assess the effectiveness of service delivery, methodologies, and progress on outcomes based on measurable and observable criteria for identifying when desired outcomes have been achieved, in line with section 245D.07 requirements.

Collaborate with physicians, patients, families, and healthcare staff.

Develop individualized care plans.

Create goals and monitor progress toward goals.

Recruit and train staff.

Create schedules.

Qualifications

Previous experience as a Designated Coordinator is required.

A baccalaureate degree in a field related to human services, and one year of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or An associate degree in a field related to human services, and two years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or A diploma in a field related to human services from an accredited postsecondary institution and three years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older; or A minimum of 50 hours of education and training related to human services and disabilities; and or four years of full-time work experience providing direct care services to persons with disabilities or persons age 65 and older under the supervision of a staff person who meets the qualifications identified in clauses (1) to (3).
