Principle Accountabilities:

Attend and actively participate in hospital multidisciplinary rounds on all patients which the hospitalist service is the primary service and when appropriate or requested when the hospitalist service is in the co-management or consultative roleCover Code Blue and Rapid Responses in accordance with the campus code committee’s recommendationsProvide admission services for the Emergency Department, incoming transfer patients, and direct or emergent admissions in accordance with the hospital on call schedule and patient assignment guidelinesProvide co-management services for subspecialty services when such services are within the scope and designation of privileges for the hospitalist ProviderProvide consultative services for members of the Medical Staff when such services are within the scope and designation of privileges for the hospitalist ProviderAccept transfers of care from other clinical service lines if the patient is deemed appropriate for the hospitalist service provided there is a physician to physician hand off communication related to the patient’s clinical course and the hospitalist attending accepts the patient in transferProvide transfer center call for incoming non-critical care medical patients in accordance with the hospital’s on call scheduleProvide timely evaluation of patients admitted to the hospital in accordance Medical Executive Committee bylawsAbide and conform to Hospital tenants, guidelines, and bylawsProvide physician to physician sign out for patients who are transitioning to post-acute facilitiesProvide internal sign out at transitions of care consistent with standards of careProvide 72 hours of narcotic prescriptions for patients transitioning to skilled nursing facilities if clinically appropriate;Participate in nursing and staff education when solicited by CNO, CMO, or COORespond to clinical queries in a timely fashionDocument History and Physicals within 12 hoursDocument Discharge Summaries within 48 hours of discharge order;Maintain current understanding of the regulatory environment related to status and status patients consistent with national guidelinesProvide post discharge coverage for appropriate home health orders for up to 2 weeks or until they have seen their primary care physiciansEncourage in network utilization of resources for continuity of care unless specified by the patient or expertise is not available in the systemRespond in a timely manner to pages and call back from nursing staff unless doing so would impact patient safety or experienceIn case of emergencies or disaster provide additional coverage per campus protocolParticipate in continuing education activities to maintain or enhance knowledge and skills;Maintain ACLS certificationDirects patient care throughout the health care system via Consult Orders.Provides consultation services in collaboration with attending or primary care provider.Communicates with the attending physician, nurse, family, and other treatment team members when appropriate, in order to obtain the appropriate patient care plan.Effectively communicates with treatment team in a timely and professional manner to provide quality psychiatric care to patients throughout the healthcare system.Complies with best practices and other care directives.Follows Performance Improvement initiatives.Works with Memorial Hermann Physicians, Risk Managers, and Case Managers towards resolution of clinical, administrative or technical problems.Maintains patient confidentiality and adheres to MH compliance privacy policies.Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.Lead Physician Specific Duties:The Lead Physician is the medical leadership representative for the Hospital Medicine Program, and performs the general administrative duties for the Program at the Hospital. The Lead Physician shall devote adequate time and his/her best efforts to properly fulfill such duties. These duties shall include:Devote adequate time to administrative duties.Assist Hospital management with preparation for, and conduct of, any inspections and on-site surveys of Hospital conducted by governmental agencies, accrediting organizations, or payers contracting with Hospital.Hold regular, at least monthly, documented meetings with Hospital providers to review care systems, patient flow, quality improvement and patient and physician relationships.Meet monthly or as often as needed with the CNO or designee to discuss planning issues, quality improvement activities, and patient care concerns.At the Hospital's request, Physician shall assist Hospital's administration in the development of operating and capital expenditure budgets for the proper and efficient operation of the Hospital Medicine Program.Work cooperatively and supportively with Hospital Administration and physicians to assure services are available, cost effective, meeting quality and regulatory guidelines.Be readily available for telephone consultation when not performing clinical or administrative tasks. Physician will designate an alternate or alternates to perform administrative functions when not available so that appropriate direction for the Hospital Medicine Program is available at all times.Assist Hospital in developing and implementing care guides that are consistent with local and national standards and other efficiency tools for inpatient services, including protocols to provide care that can eliminate the need for marginal admissions.Attend meetings, participate in quality assurance and quality improvement (“QA/Q1”) activities, and educational functions to evaluate and maintain the quality of care of the Hospital Medicine Program.Serves as an advisor to the Hospital's Utilization Management, Case Management Risk Management and Quality Management Departments.Directs hospitalists care coordination to facilitate expeditious and appropriate patient flow management and utilization.Cooperate with Hospital in the preparation of claim forms for reimbursement, and of other appropriate reports on the operation of the Hospital.Oversees and evaluates hospitalist team members’ clinical, interpersonal, and operational performance, and provides feedback and recommendations to the Medical Staff, campus executives, and Hospitalist Program Management Office regarding hospitalist performance.Address performance issues and provide coaching and development support directly to the hospitalist.Coordinate membership of hospitalist physicians on various assigned committeesParticipate on Hospital and Medical Staff committees at the request of the Administrator or the Medical Staff, and attend the Department of Medicine and other Medical Staff committees as appointed and/or elected.Make recommendations and assist Hospital in promoting good community relations and public recognition for the hospitalist program.Perform such other responsibilities as reasonably requested by the Hospital or the Medical Staff.Develop and review and amend if necessary a disaster plan for the hospitalist service in coordination with hospital and System wide plans.Develop clinical protocols and guidelines for the hospitalist program in coordination with campus efforts and system led initiatives.Oversee recruiting efforts for the Group.Oversee the credentialing process for new hires.Monitor quality and appropriateness of medical care provided by the hospitalist service.Develop, maintain, and update policies and procedures manual for the group practiceDevelop and maintain an internal clinical review process for the hospitalist group in coordination and consistent with the hospital clinical peer review process.Be available to respond to and or address clinical patient complaints.Develop, maintain, and update co-management agreements/guidelines where appropriate between the hospitalist program and subspecialty physicians or groups.Attend, or on occasion send a designee to attend MHMD Clinical Programs Committee (“CPC”) meetings for hospitalists and assist with the implementation of evidence-based protocols approved by the CPC.Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.Ensures safe care to patients, staff and visitors; adheres to all Memorial Hermann policies, procedures, and standards within budgetary specifications including time management, supply management, productivity and quality of service.Promotes individual professional growth and development by meeting requirements for mandatory/continuing education and skills competency; supports department-based goals which contribute to the success of the organization; serves as preceptor, mentor and resource to less experienced staff.Other duties as assigned.
