Starting in northeast Rochester, NY, Jordan Health has since expanded to become a network of primary care offices and health centers serving residents living in three quadrants ofRochester, NY (http://www.city-data.com/city/Rochester-New-York.html) and in Canandaigua, N.Y.Jordan Health is now seeking aRegistered Dietitian Nutritionist, Certified Diabetes Educatorwho, under the guidance of the Director of Social Emotional and Mental Health, is primarily responsible for providing nutrition counseling and assists in developing nutrition programs for patients with specific medical or nutrition-related problems such as obesity, diabetes, hypertension, lipid disorders, food allergies, etc. Identifies problems associated with inappropriate dietary intake, such as poor pregnancy outcomes, food insecurity, treatment side effects and/or other related problems.If you welcome the prospect of working in an environment where creativity and openness to trying new techniques is welcomed, and where you can support teams whose mission is to significantly impact our patients quality of life, please consider ourRegistered Dietitian Nutritionist, Certified Diabetes Educatoropportunity.RequirementsTheRegistered Dietitian Nutritionist,Certified Diabetes Educatorwill ensure to:Collaborate with providers to identify and connect with patients in need of nutrition counselingReview patients medical charts for pertinent laboratory values, clinical assessments, anthropometric data and other information that serves as the basis for development of patients nutritional care plans.Consults with health care providers regarding lab test, return visits and referrals to other professionals within Jordan Health as needed, and cooperates in the coordination of patient care.Participates in the development and planning of educational programs for groups of patients with specific nutrition-related concerns (i.e.: diabetes, hypertension, hypercholesterolemia, etc.)Participates in quality improvement measures of nutritional services.Required Qualifications:Bachelor Degree in Nutrition Management or related fieldExperience working with diverse populations in ambulatory health care settingLicenses and Certifications:Current certification as a Registered Dietician Nutritionist by the Commission on Dietetic Registration required.Certified as a Diabetes Educator preferred.Infection Control certification.BLS through the American Heart Association.Special Skills, Knowledge RequiredExcellent oral and written communications skills, basic computer skills, and experience using Electronic Healthy Record is required.Understanding of the Nutritional Care Process for patients with diabetes, cardiovascular disease, hypertension, infectious disease, obesity, and pregnancy.The ability to relate to patients of different ages, varying educational levels, and ethnic backgrounds.Must be interested in and willing to work with migrant and refugee populations.Requires enthusiasm for the subject, maturity, flexibility, willingness to be creative and openness to try new techniques and approaches.Willingness to update professional competencies by attending workshops and conferences pertaining to nutrition/health related issues.Benefits: Jordan Health offers a competitive salary and full benefits offering including medical, dental, vision, life insurance, and 403b retirement plan.
