Role Summary:As a Primary Care Physician, you'll conduct in-person, comprehensive assessments and collaborate with the IDT to craft personalized care plans for each participant. Under the Medical Director's guidance, you will manage the entirety of participants' care in various settings, including nursing homes and hospitals. This encompasses overseeing admissions and discharges across all care levels and coordinating with medical specialists for optimal care continuity. Additionally, you'll provide leadership and support to Nurse Practitioners and other IDT members.
Key Responsibilities:Perform assessments, primary care, and plan ongoing medical management for participants.Facilitate smooth transitions from acute or long-term care settings in collaboration with external physicians and the IDT.Engage actively in IDT meetings, sharing insights on participant conditions, making care planning decisions, and ensuring round-the-clock care coordination.Regularly visit nursing home residents based on individual needs.Participate in the on-call rotation during nights and weekends.Qualifications:
Residency completion in a program accredited by the Accreditation Council for Graduate Medical Education.Master's/Doctorate Degree in Medicine.Board Certification in Internal Medicine or Family Practice, with a preference for Geriatrics certification.Unrestricted California Physician License.State CDS Certification and DEA Registration.Why Join Us?
