Interview and assess all residents prior to the initial Care Plan Conference; document this information in the medical record, develop an individual recreation plan based on the assessment and participate in Interdisciplinary Care Plan meetingsUpdate assessments and plans as needed and required by state or federal regulationsDevelop monthly recreation program calendars that reflect and meet the needs of facility residentsCommunicate facility programs to residents, staff, family and volunteersManage facility Volunteer ProgramMaintain departmental documentation that reflects services provided and resident progress towards goalsIn coordination with social services facilitate the residents in the organization and continued development of a Resident's CouncilMake job assignments and set prioritiesServe as member of QAA committeeQualifications:
As required by the state, currently licensed or registered in the state of practice as Activities Director, Social Worker, Occupational Therapist (OT) or Occupational Therapy Assistant (OTA),Minimum two (2) years' experience in social or recreational program within the last five (5) yearsMinimum one (1) year full-time experience in health care activities programCertification as a Therapeutic Recreation Specialist preferred
