The mission of the Department of Internal Medicine, Division of Geriatrics is to practice the highest quality, safe and cost effective adult care in acomprehensive, innovative environment which is individualized and sensitive toa diverse population. The Departments goal is to enhance scientific knowledge and carethrough creative and pioneering investigation and to provide comprehensiveeducation and training to students and all professionals with enthusiasm andrespect.
Chief Purpose of this position:
TheDivision of Geriatric Medicine at the Virginia Commonwealth University supportsa large health system that cares for many adult patients with advanced chronicillness. The Division seeks a new clinician educator to advance the academic program. TheVCU Center for Advanced Health Management (CAHM) program is a multi-facetedcare system including; office-based medical care for those who arecomparatively mobile; home-based care both short-term and long-term for thosethat are less mobile; nursing home care for both post-hospital rehab and longterm care; inpatient consult service; behavioral health counseling; pharmacyconsultation; fully developed transitional RPM and telemedicine services(triage, counseling, monitoring, video-visits); and care management and socialwork services. This faculty member will treatpatients as part of the Nursing Facility Attending Service (NFAS) where the MDdirects and organizes a complex range of medical and other care services thatsupport a patient's recovery from surgery, illness or management of a chronicillness or disability in a nursing home. The NFAS is comprised of four MD+APPteams in the Richmond market which combine to care for >90% of dischargedpatients managed in the VCUMC preferred SNF continuing care network. This includes patients were VCUMC carriesfinancial risk such as indigent care contracts, the CMS bundled payment model, and theCMS value-based purchasing arrangement that incorporates readmission reductiongoals. This consistent VCUHS care achieves optimal levels of performance and outcomesby organizing and orchestrating a wide variety of services all incorporatingbroad inter-disciplinary teamwork.
Position Responsibilities:
Teaching: To support teaching directly via interactions with Fellows, residents, medical students, and non-physician professionals. (Affiliate faculty appointment)
Research: To support the research mission of the Division.
Service: To provide service of the highest quality to the people of the state of Virginia and beyond.
Clinical: Anticipated clinical effort is 90%.The workload and time commitments are commensurate with existing Geriatrics faculty.
Duties: To be responsible for managing a panel ofpatients throughout the continuum of care. To assure that care is timely, appropriate, of high quality, and costeffective. To create and enhance systems to assure thatall patients receive planned, evidence-based care and care is fully coordinatedwith ongoing continuity for the patient, family and caregiver. To workcollaboratively with Attendings, Fellows, Residents, NPs, and other staff toensure a team-based approach to care is fostered. To understand and welcome the importance ofroles as care providers, team members, and major influencers of our community'sperception of the geriatrician's quality and service. Sharing night call (onein nine currently).
Other: Submit billing and document care as required by the organization (estimated to bill 8 encounters per day, billing all payer types with timely and thorough documentation in the EPIC record) and to abide by all medical staff requirements. Participate in quality initiatives.
Required Qualifications:Board certified/board eligible in Internal MedicineMust be comfortable working with an interdisciplinary team including nurse practitioners and pharmacists, and must integrate well with members of an extended care teamMust have direct experience managing complex inpatient and outpatient geriatric patients with a solid understanding of the geriatrician role and functionDemonstrated experience working in and fostering a diverse faculty, staff, and student environment or commitment to do so as a faculty member at VCUPreferred Qualifications:
Geriatric fellowship training strongly preferredExperience with EPIC electronic health record preferred
