Job DescriptionWorks closely with the clinical and wellness coordinator to ensure that PHQ9 screenings are completed and re-assess for connection to care as needed. (in-person)Continues open communication between supervisor; direct care staff members; family members; guardians; collateral individuals regarding the individual's status/progress, Clinical oversight of the clinical and wellness coordinator.Provides a tentative schedule to supervisor of appointments and reports weekly on crisis intervention and behavioral sessions. Create a service plan to be implemented with members who are in clinical mental health care.Participates in Interdisciplinary Team Meetings as well as other pertinent meetings/appointments to ensure all team members are continuing open communication of the individual's progressCompletes monthly/quarterly progress notes as well as attend monthly/quarterly/MHI meetings with funder for professional development and coaching and with the director and Clinical and Wellness Coordinator to discuss individual's progressConducts training/in-service trainings, annually, and as needed to ensure staff and other team members are competent on the implementation of the program plan and/or documentation sheetsProvides follow-up and reports observations for continued support for the individual's or family needs .Have effective record keeping (i.e., monthly/quarterly notes; in-service training; contact information; program plans; etc.) to ensure information is readily available to interested parties (i.e., management; surveyor; etc.)Assists in developing procedures for training materials for staff serving members for the purpose of enhancing programs for individuals and ensuring that program operations follow established guidelines.Continued education on behaviorally based interventions & techniques to stay competitive in the field.Follow-up and PHQ9 initial assessment and re-assessment as needed for members who score 10 or higher. Include setting a safety plan with members who are high risk or reporting a history of self-harm.Performs other related duties as assigned for the purpose of ensuring the efficient and effective functioning within the work unit.Qualifications
Must be licensed or provisionally licensed in State of New York, LMSWMaster's degree in social work, counseling or closely related field (behavioral health);
LMHC - Licensed Mental Health Clinician; LMSW - Licensed Master Social Worker; • LCSW - Licensed Clinical Social Worker1-2 years' experience working in a behavioral health setting preferred.Excellent organizational, communication, problem solving, interpersonal, and customer service skills.Flexible with work schedule and job duties with the ability to effectively meet changing priorities and deadlines.Adept in problem solving skills and able to work in a stressful, fast-paced environment.Ability to handle confidential information in a professional manner.Ability to interact with people of all ages in a supportive manner.management and structure, and regulatory and licensing standards, with ability to effectively implement them at an agency and program level.Must be able to effectively utilize Microsoft Word, Excel, and other required computer programs.Minimum age requirement is 21 years.Must meet all agency requirements for pre-employment as required Equus Workforce Solutions and/or state regulations.
