Job Type Full-timeDescriptionMental Health Practitioner - ARMHSPeople Incorporated Mental Health Services is seeking a full-time Mental Health Practitioner I or II to join our Integrated ARMHS team. This team provides direct mental health services that are rehabilitative and enables the recipient to develop enhanced psychiatric stability, social competencies, personal and emotional adjustment, community, and independent living skills when these abilities are impaired by symptoms of mental illness. This is executed by exercising a person-centered approach to client support and services, by performing assessments, providing education, promoting healthy lifestyles, facilitating individual meetings, clear and timely documentation and delivering individualized plan of care consistent with client goals and objectives.People Incorporated Integrated ARMHS team provides Adult Rehabilitative Mental Health Services to individuals receiving services from People Incorporated Outpatient Mental Health Clinics, Targeted Case Management teams, and outreach teams. The Integrated ARMHS team additionally provides Transition to Community Living Services to individuals residing in People Incorporated IRTS programs. The ARMHS team emphasizes integrated care as part of our Certified Community Behavioral Health Clinic (CCBHC) which is designed to ensure access to comprehensive behavioral health care.Schedule: Full-time, Monday - Friday Business Hours. Some remote work, flexible schedule within daytime hours.Location: Most services are provided in Hennepin and Ramsey COHiring Range: $20.83 - $24.44, depending on qualifications. For over 50 years, we have been a leading community partner, transforming the health of our communities through innovative solutions.Main Job Duties:Using person-centered principles, actively engage clients in their mental health recovery, and develop and maintain appropriate relationships that support the client in meeting their individual goals.Facilitate and conduct individual services that address a variety of needs and health concerns for the client. May facilitate and conduct group interventions, if applicable.Provide collateral information for nursing assessments and conduct interviews and gather information to inform treatment planning, plan-of-care reviews and discharge plans as needed or directed.Document interactions and interventions with clients, according to program requirements, into the organization's Electronic Health Record (EHR) and/or record-keeping system.Provide crisis intervention and de-escalation for clients experiencing mental health symptoms or other emotional and/or behavioral responses while prioritizing client and program safety.Participate in coordination of services through the recognition of the multiple needs of the client, with other providers in the community, in an effort to provide necessary resources to the client.
