Job Description
Stamford Hospital's prestigious pharmacy team is seeking a per-diem pharmacist to join the mission and ensure effective delivery of pharmaceutical care to all our patients. This pharmacist will report to the Director of Pharmacy and are responsible for providing quality pharmaceutical care with an emphasis on accuracy and patient safety. The right incumbent for this role will demonstrate the knowledge and skills necessary to provide comprehensive, person-centered pharmaceutical services. These services include medication delivery, therapeutic recommendations, and promotion of optimal drug therapy through policy development.Per-Diem! A minimum of 4 shifts per month is required. Daytime and evening availability is a must!Hospital experience required!

Responsibilities
Accurately verify, prepare and dispense medication orders per physician request according to established protocols.Provide timely and accurate drug information to healthcare professionals and lay persons.Participate in medication counseling sessions for patients by reviewing side effects, indications and directions for use.Ensure appropriateness of therapy and dosage for all patient populations including age-specific careMonitor patient therapy in accordance with departmental policies and procedures.Carefully identify, prevent, intervene, and follow up on any medication related problems.Supervise pharmacy support personnel to ensure the seamless delivery of pharmacy services throughout the institution.
Qualifications
Bachelor of Science in Pharmacy required. Advanced degree preferred.
Current license or eligible for license as a Registered Pharmacist in Connecticut required
