Essential Function: The RN Case Manager facilitates and coordinates the patient's treatment plan of care to assure quality through the continuum of care. Collaborates with the interdisciplinary team to implement the plan of care and transition strategies, ensuring the achievement of desirable patient outcomes, appropriate length of stay, efficient utilization of resources, increased patient and family involvement, and patient/family/staff education. Develops individualized care plans, including prioritized goals, that consider the patient's and caregivers' goals, preferences, and desired level of involvement in the care management plan.Makes outbound calls to assess patient's current health status.Identifies gaps or barriers in treatment plans.Coordinates care for members and makes referrals to outside sources.Ensures members discharged from hospital or emergency department receive the necessary services and resources, including medication reconciliation.Triages symptom-based calls per approved protocols for a wide range of patients.Enrolls patients and manages alerts for various home monitoring programs.Provides approved general health information and guidance to public callers from approved sources by assessing their needs.Documents all workflows in EPIC system for tracking and quality assessment.Provides patient education to assist with self-management of wellness and disease.Educates members on chronic and acute disease processes.Forms partnerships with providers to achieve quality and cost management objectives.Tracks and monitors enrolled members throughout the continuum of care.Interacts with LCMC Healthcare Partners leadership and/or physician group partners to discuss clinical questions and concerns regarding specific cases.Participates in case conferences with the interdisciplinary team to strategize and develop member specific care plans.EXPERIENCE QUALIFICATIONS2-3 years of Nursing experience requiredEDUCATION QUALIFICATIONSRequired: Bachelor's Degree in Nursing or equivalentLICENSES AND CERTIFICATIONSCertification Name: Basic Life Support Health Care ProviderRequiredIssuer: American Heart AssociationLicensure Specialty: Training CertificationCertification Name: Certified Case ManagerPreferredIssuer: Commission for Case Manager CertificationLicensure Specialty: Specialty CertificationCertification Name: Registered Nursing LicenseRequiredIssuer: Louisiana State Board of NursingLicensure Specialty: LicensureThe above job summary is intended to describe the general nature and level of the work being performed by people assigned to this work.
